A vacancy exist for a Nurse Practitioner for Primary Care Service in our PACT team at the Dayton VA Medical Center.
POSITION SUMMARY: The
Patient Aligned Care Team (PACT) Nurse Practitioner (NP) works in partnership with their Core Teamlet (Teamlet made up of Veteran, Primary Care
Provider (PCP), RN Care Manager, LPN, and Medical Support Assistant), Tier II
disciplines (Pharmacist, Social Worker, Dietician, Behavioral Health) and
Expanded Team members (family/caregiver, internal and community-based services)
in providing effective disease prevention/health promotion and chronic disease
care management of their assigned panel of Veterans. The PACT NP uses the nursing process and
evidence-based practice to collaborate with others in managing their assigned
panel of patients through the application of specialized, advanced clinical
health care knowledge and skills in the areas of direct clinical care,
consultation, education of Veterans, families and personnel in the Primary Care
Prescriptive Authority is Mandatory.
