This position functions in the Program Support Assistant (Allied Health Coach Trainee), Primary Care Line Service.  Incumbent will meet with Veterans in individual and group settings to provided education on the process for developing a Personalized Health Plan.   Duties may include, but are not limited to:

·         Incumbent will be responsible for tracking contact hours with Veterans and ensuring appropriate follow up to support the Veteran in achieving their health goals.

·         Steps involved in creating a personalized health Plan include: completion of an initial Health and Wellness Assessment, completion of a Personalized Health Plan that outline the Veteran's behavioral health goals; and follow up health coaching sessions to build skills and confidence in executing the plan.

·         Incumbent will work with the care team to select appropriate Veterans to participate in this pilot program.

·         Incumbent will be expected to have basic administrative skills including typing, computer software skills (i.e. Microsoft Office package), emailing (i.e. Microsoft Outlook) and telephone skills.

·          Incumbent should have some familiarity with healthcare delivery and basic behavioral health coaching skills either through educational experience or vocational training but not is not expected to provide expert medical advice to Veterans.

To qualify for this position: applicants must meet all requirements by the closing date of this announcement.  


You must be a student currently enrolled at least half-time in one of the following disciplines:  Social Work, Psychology, Dietetics, Physical Rehab, or Nursing.


You must also have a GPA (Grade Point Average) of at least 2.0 on a 4.0 scale.


For GS-4:  One year of General Experience, 2 years above high school or a combination of General Experience and education.  General Experience is progressively responsible clerical, office, or other work that indicates ability to acquire the particular knowledge and skills needed to perform the duties of the position to be filled.


For GS-5:  One year of Specialized Experience equivalent to at least the GS-4 level.  Specialized Experience is experience that equipped the applicant with the particular knowledge, skills, and abilities (KSA's) to perform successfully the duties of the position, and this is typically in or related to the position to be filled.  To be creditable, Specialized Experience must have been equivalent to at least the next lower grade level.

Functional Requirements:  Heavy lifting (45 pounds and over); heavy carrying (45 pounds and over); straight pulling (2 hours); pushing (4 hours); reaching above shoulder; use of fingers; both hands required; walking (6 hours); standing (2 hours); repeated bending (2 hours); both legs requires; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13' to 16' to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish shades of colors; hearing (aid permitted).

Environmental Factors:  Outside; excessive noise (intermittent); slippery or uneven walking surfaces; working with hands in water; working closely with others; protracted or irregular hours of work.
