You will be responsible to provide pharmaceutical care to patients seen in the ambulatory setting at the Community Based Outpatient Clinic (CBOC) in Sierra Vista. You will consistently strive to provide pharmaceutical care to patients and optimize patient care outcomes. Supervision of Pharmacy Practice Residents and Doctor of Pharmacy students is inherently part of the responsibilities. A limited portion of the responsibilities will include but not be limited to:

·         Plans and conduct research projects. 

·         Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria, where applicable.

·         Actively be involved in reviewing patient medication regimen for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions and therapeutic outcomes as required and communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. 

·         Applies knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.

·         Must have expertise in the principles of clinical pharmacokinetics and pharmacodynamics and knowledge to make dose adjustment recommendations based on objective laboratory findings. 

·         Reviews and implement Pharmacotherapy and treatment guidelines with providers;

·         Documents clinical interventions accurately; and, work collaboratively with clinical providers to provide pharmaceutical care to selected patients.

·         Serves as an authority on drug usage, interactions, over-dosages, and adherence to medical staff, nursing staff, technicians and students. 

·         Must have a good working knowledge of information resources, both written and electronic.

·         Provides timely and accurate responses to drug information inquires from customers. 

·         Monitors and report drug errors, adverse drug reactions, allergies, and patient adherence issues. 

·         Routinely review literature to provide an evaluation of efficacy, safety, and cost-effectiveness for routine patient care and projects assigned. 

·         Promotes and monitors compliance for drug therapy policies.

·         Works with providers to ensure compliance with national, VISN, and local initiatives.

·         Serves on medical center and/or VISN committees as requested. 

·         Serves as an effective preceptor, directing and supervising pharmacy residents and students as assigned.

·         Assists in the development, enhancement, and operation of resident and student training programs; and, participate in activities of interdisciplinary training and in-servicing of medical staff, nursing staff, residents, students and other allied health professionals at SAVAHCS. 

·         Effectively instructs patients and family members in the appropriate use of medications. 

·         Assists in planning and implementing of Pharmacy quality assurance activities. 

·         Assists in therapeutic recommendations for selected disease states according to best evidence or VHA and/or SAVAHCS published guidelines. 

·         Assists in the operations of the other areas of Pharmacy Service Line including distribution and clinical work in other areas.

·         Responsible for guidance and direction of staff members as needed by circumstance, making appointments for patients as required following all VA rules and regulations, and assisting in designing and completing research studies involving clinical pharmacist functions. 

·         Participates in collaborative research studies involving drug therapy with other services. 

·         Supervises projects as required by pharmacy residents and students.


Work Schedule: Monday thru Friday – 8:00 a.m. to 4:30 p.m.

Relocation Expenses: Relocation expenses ARE NOT authorized for this position.

QUALIFICATION REQUIREMENTS:  VA Qualification Standards Handbook 5005, Part II, Appendix G15 for Licensed Pharmacist.

    •   Graduate of a degree program in pharmacy from an approved college or university. MINIMUM QUALIFICATION REQUIREMENTS: GS-13: Completion of the equivalent of 1 year of creditable experience at the next lower grade level GS-12, which is directly related to the position to be filled.

Specialized experience is experience that equipped the applicant with the particular knowledge, skills, and abilities (KSAs) to perform the duties of the position. 

Examples of qualifying specialized experience would include:

• Practicing in an ambulatory care specialty (e.g., medicine, surgery, psychiatry, neurology, spinal cord injury, ambulatory care, rehabilitation medicine, geriatrics, medical research).

You will be rated on the following Knowledge, Skills, Abilities and Other characteristics (KSAs) based on your application for this position:

KSA -1: Knowledge of a specialized area of clinical pharmacy practice or a specialty area of pharmacy such as quality assurance/utilization review, ADP systems, drug information.

 (2)  Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). 

PHYSICALS REQUIREMENTS:  This position may require lifting of 15-44 pounds, walking and standing of 2 - 4 hours, reaching above the shoulders, and use of fingers.
