This position
is being filled under the Pathways Internship Program. 
The program is designed to provide students enrolled in a wide variety of educational institutions, from high school to graduate level, with paid work opportunities in agencies to explore Federal careers while still in school.  The Internship provides manager's a ready source of student employees and potential permanent employees upon completion of the program requirements.  It also provides the student with work experience directly related to the curriculum studies or career goals while offering eligibility for non-competitive conversion to permanent employment upon completion.  For more information on Internship opportunities within the Department of Veterans Affairs, please visit http://MyCareeratVA.va.gov/CareerPath/Internships

Pathways temporary student position located in Primary Care Service for the Dayton VA Medical Center at the Middletown, Ohio Community Based Outpatient Clinic.  Primary responsibility includes meeting with Veterans in individual and group settings to provided education on the process for developing a Personalized Health Plan. 

Your duties may include, but are not limited to, the following:

· Trainees would help support Personalized Health Plan pilot teams by providing health coaching services to Veterans with active personalized health plans.

· Health Coaching services would primarily be delivered via telephone visits and secure messaging with the patient.

· Services would focus on developing self-management skills, building confidence in managing health and providing overall support for the Veterans health goals.  

 Work Schedule:8-4:30 Mon-Friday

Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Internship Program:  To be eligible, applicants must be accepted for enrollment or currently enrolled and seeking a degree (diploma, certificate, etc.) in a qualifying educational institution, on a full or half-time basis.

Interns may be converted to a permanent position (or, in some limited circumstances, to a term position lasting 1-4 years) within 120 days of successful completion of the program. To be eligible for conversion, Interns must:

  • Complete at least 640 hours of work experience acquired through the Internship Program

  • Complete their degree or certificate requirements

  • Meet the qualification standards for the position to which the Intern will be converted

  • Meet agency-specific requirements as specified in the Participant's Agreement, and perform their job sucessfully

Agencies may waive up to 320 of the required 640 hours of work for Interns who demonstrate high potential as evidenced by outstanding academic achievement and exceptional job performance.

In addition, students working in agencies through third-party intern providers may count up to 320 of the hours they work toward the 640 hour requirement.

Time spent under previous Internship Program appointments may count towards required work experience hours


EDUCATION:

Two (2) years of education above the high school level. This education must have been obtained in an accredited business or technical school, junior college, college or university. One year of full-time academic study is defined as 30 semester hours, 45 quarter hours, or the equivalent in a college or university, or at least 20 hours of classroom instruction per week for approximately 36 weeks in a business, secretarial, or technical school in the following areas of study:

o Social Work

o Psychology

o Dietetics

o Physical Rehab

o Nursing

Allied Health

Transcripts must have be submitted with your application if you are basing all or part of your qualifications on education.

Equivalent combinations of post-high school education and general experience to meet the total requirements.

You will be rated on the following Knowledge, Skills, Abilities and Other characteristics (KSAOs) based on your application for this position.

1. Provide education to Veterans on process for developing a Personalized Health Plan (PHP)

2. Tracking contact hours with Veterans on their PHP

3. Work with Teamlet to select appropriate Veterans to participate with the PHP.

4. Ability to communicate effectively, both orally and in writing, with people from a variety of backgrounds and levels of understanding.

5. Data collection for outcomes from PHP program
