The Senior Social Worker practices in the Health Care for Homeless Veterans (HCHV) Program and will be based at a Community Based Outpatient Clinic (CBOC), however, will provide services to clients primarily in the community serving a patient population with highly complex health and mental health problems, and serves as a Safe Haven Liaison in the HCHV program.

Specific duties of this position include, however are not limited to; functioning as case coordinator and liaison for Veterans in the HCHV Safe Haven program and collaborating with other members of the HCHV team, the Grant and Per Diem programs, the VA Supportive Housing (VASH) program and other professional specialty clinics to insure comprehensive care to Veterans in the HCHV program.

The Senior Social worker effectively provides case coordination services including ongoing assessment, education, coaching, counseling, referral and advocacy. The incumbent will work with a designated community organization responsible for the case management and transitional housing of Veterans to assure all admissions, discharges, and treatment goal planning are in alignment with the stated agreement. The incumbent will insure that Housing First and Harm Reduction modalities are being practiced in administration of the program.

The incumbent also refers Veterans to appropriate VA and other government/community services including local shelters, food banks, employment services, and Veteran's benefits. Based on the changing needs of the Veterans and their responses to interventions, the Senior Social Worker makes adjustments to the psychosocial treatment plan and interventions. The incumbent initiates and effects changes in methods and interventions to promote efficient practice and improve patient outcomes.

 

 

 

 

 

Work Schedule: Monday-Friday, 8AM-4:30PM.  May have weekend work, holiday, and/or extended hours.

Relocation expenses will not be reimbursed.

BASIC REQUIREMENTS: 

  • U.S. citizenship. 
  • Proficient in spoken and written English.
  • Posses a valid, current, and unrestricted driver's license.  Upon selection, must be found suitable to transport patients. 
  • Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (Transcripts required).
  • A license or certification by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination; grandfathered by the state in which they are licensed to practice at the advanced practice level; licensed by the state of California, which administers its own clinical examination for advanced practice; have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. (Must provide proof of licensure). 

 

SPECIALIZED EXPERIENCE FOR GS-12 LEVEL: Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment.  At least two years of advanced practice are in an area of specialized social work practice.  Examples of advanced practice level may include providing independent and expert clinical psychosocial and case management services in a specialized area of practice to veterans who tend to have serious and severe crises.  Experience must have been obtained AFTER earning a Master's Degree in Social Work.

 

The Professional Standards Board will seek the following in a best qualifying candidate:

1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness.  Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.  Ability to determine priority for services and provide specialized treatment services. 

2. Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations.  This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with polytraumatic injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, etc. 

3. Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services.  Ability to coordinate the delivery of specialized psychosocial services and programs.  Ability to design system changes based on empirical findings. 

4. Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.  Ability to teach and mentor staff and students in the specialty area of practice and to provide supervision for licensure or for specialty certifications.

5. Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs.  Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.

Receiving Service Credit for Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
