One full-time Registered Nurse is available in Mental Health & Behavioral Sciences Service, Housing Urban Development/VA Supported Housing (HUD/VASH) Program.  The incumbent will serve as a Case Manager.  HUD-VASH is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services while residing in stable housing through HUD’s Section 8 voucher system. Tour of duty will be Monday through Friday, 8am to 4:30pm.  There may be some instances where the incumbent would be required to work after 4:30pm, or have the flexibility to work outside the normal tour of duty which may include weekends and holidays.  This position requires travel into the community in support of HUD-VASH. Travel may be required approximately 50% of duty time.  This announcement is open to qualified U. S. Duties and responsibilities include but are not limited to the following:

a. Executes position responsibilities that demonstrate experience and creative approaches to clinical care management of homeless veterans.

b. Functions in a direct patient care capacity as related to individual and group treatment interventions.

c. Provides direct patient care to address medical and psychosocial needs. Responsibilities include triaging, assessing, planning, and assisting with treatment interventions as related to mental health problems, health promotion, and preventive care. This also includes providing patient education as well as monitoring pharmacological effectiveness and medication side effects.

d. Engages in community outreach services at various community homeless sites to provide clinical oversight and case management for homeless veterans in the community or in a medically convalescing setting. This will include a clinical assessment, inventory of patient needs, and provision of health education. It is also expected that the RN will oversee and be responsible for the drug screen collection and monitoring procedures for homeless veterans referred for care.

e. Ensures that the practice of nursing is consistent with The Joint Commission, CARF, VA policies and procedures, clinical practice psychiatric guidelines, VISN standards, national statues and regulations, medical record documentation, mandatory continuing education, and HIPPA policies.

f. Inputs workload by encounters as well as completes all Form Xs in a timely manner.

Collaborates with members of the multidisciplinary team so as to develop and implement an individualized treatment plan and program to meet the needs of the homeless veteran.

g. Exercises professional customer service skills with a courteous demeanor, excellent organizational skills, and a problem solution approach with community agencies and organizations that care for the homeless veteran.

h. Demonstrates ability to establish and maintain effective communication and interpersonal relationships with members of the interdisciplinary team, patients, families, visitors, and staff.

i. Demonstrates leadership in delivering and providing holistic care, utilizing collaborative strategies with patients, family members/significant others and members of the interdisciplinary team.

j. The HUD VASH nurse participates in planning for program improvements at the local level, and in collaboration with the Interdisciplinary Team, is responsible for coordinating performance improvement initiatives and for collecting relevant data required for the Commission on Accreditation of Rehabilitation Facilities (CARF) certification and Joint Commission on the Accreditation of HealthCare Organizations (JCAHO) standards.

k. Performs other directly related duties as required.

Basic Requirements:

Citizenship.  Citizen of the United States.  (Non citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)

Graduation from School of Nursing

Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed by the applicant.  [NOTE:  See exception in subparagraph (b) below for candidates who are enrolled in a MSN Bridge Program.] (1)The National League for Nursing Accrediting Commission (NLNAC). OR (2)  The Commission on Collegiate Nursing Education (CCNE). 

[The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing.  

In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration (see paragraph 2a(3)) will meet the requirement of graduation from an approved school of professional nursing.

Registration:

Condition of Employment.  A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.

Physical Standards.  Per VA Handbook 5019, Part IV, Appendix A, all full- and part-time employees, Volunteers and trainees are required to complete a Tuberculin Test prior to employment or assignment to duties where they may be exposed to tuberculosis.

Functional (Physical) Requirements: Light lifting, under 15 pounds; Light carrying, under 15 pounds; Reaching above shoulder; Use of fingers; Both hands required; Walking (1 hour); Standing (1 hour); Both legs required; Ability for rapid mental and muscular coordination simultaneously; Both eyes required; Depth perception;  Hearing (aid permitted).

English Language Proficiency.  RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).

Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.   Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research. Education/Experience Requirements: 

·         Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.

·         Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience. 

·         Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

Other Qualifications: Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.

Preferred Qualifications: Mental health nursing experience; Case management nursing experience; and Community health, public health or home health experience.
