This position will assigned to the Hickory Outpatient Clinic serving Veterans in the Primary Care setting.  The initial orientation to the Veteran's Administration Hospital will be conducted at the Salisbury, NC  facility.  The Primary Care Registered Nurse will be responsible for, but not limited to:


•  Utilizes the nursing process and evidence-based practice to collaborate with the Primary Care team in developing the patient-driven holistic care plan for life.
•  Collaborates with patients to assess and identify needs, issues, care goals, and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery, and health maintenance/wellness.
•  Responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patient’s needs.
•  As a member of a larger team, promotes patient and family health education with a focus on self-management, prevention, and wellness, based on patient’s goals.
•  Responsible to provide both team and disease case management for a panel of patients assigned to Community Based Outpatient Clinic (CBOC) as applicable.
•  Has the ability to utilize multiple communication modalities to meet the needs of the patient including telephone, secure messenger and office visits
•  Promotes an environment that embraces new knowledge, innovations, and improvements.
•  Demonstrates leadership by serving as an advocate for patients, team leader to colleagues.
•  Travel to other Outpatient sites may be required.
•  The operational hours for the clinic include weekdays, weekends, and holidays.
