The following is a list of duties related to this position:
You will initiate routine calls/contacts with referred/enrolled patients to monitor adherence to treatment and to gather information to monitor depression and Post-Traumatic Stress Disorder (PTSD) symptoms and severity using validated tools.
- Maintain data files and records regarding individual patient progress; patient adherence to treatment; treatment modifications; communications with patient, primary care provider, and supervising psychiatrists; and operational issues arising during the program.
- Communicate routinely (weekly) with the supervising psychiatrist(s) regarding patient status and conveys any resulting recommendations to the referring primary care providers.
- You will maintain active relationship with all clinical practice staff to ensure the efficient flow of communication and paperwork associated with the program.
- Participate routinely and as requested in local and national conference calls relative to program operations and reporting requirements.
- You will serve as liaison between the patient and the health care professionals. Coordinates and follows each case ensuring that the patient is aware of all appointments.
- Conduct reviews and present findings and recommendations as necessary.

Degree: Applicants MUST have a degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
Licensure/Certification: Applicants MUST have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States
Specialized Experience: One year of experience in the same or similar work equivalent to at least the next lower level (GS-09) requiring application of the knowledge, skills, and abilities of the position being filled as a Registered Nurse. Candidates for this position must clearly demonstrate in their resume that they possess specialized experience which involves a wide range of health care practices, principles, and concepts, and extensive experience with medical practices and procedures; ability to communicate effectively with healthcare providers at all levels; knowledge in Case Management Standards.
Education Substitution: If an applicant meets the basic requirements for Education and Registration/Licensure but fails to meet the required one (1) year of specialized experience; Completion of all requirements for a doctoral degree (Ph.D. or equivalent) or 3 full years of progressively higher level graduate education can be substituted.Only degrees from an accredited college or university recognized by the Department of Education are acceptable to meet positive education requirements or to substitute education for experience. This position is subject to annual seasonal influenza vaccinations. Applicants tentatively selected for appointment to this position will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
This position is a Testing Designated Position (TDP) subject to applicant testing and random drug testing. Tentatively selected applicants will be required to sign DA Form 5019 requiring participation in random drug urinalysis testing. A job offer to an applicant who is a not a current employee will be withdrawn if the applicant refuses to be tested. Applicants with verified positive test results shall be refused employment. All Health Care Providers must be able to obtain and maintain current Basic Life Support (BLS) Training and certification Current Advanced Life Support of other advanced certification does not supersede BLS completion.This position requires a Childcare Background Check (CNACI).
