Duties

The Nurse Practitioner, Spinal Cord Injury & Disorders (SCI&D) Program is a registered, professional nurse practitioner (APRN) who provides care in a variety of settings to a clinically and psychosocially complex population of spinal cord injury and/or disorder Veterans of the VA Pacific Islands Health Care System (VA PIHCS) in Honolulu, Hawaii.  The duties of the Nurse Practitioner SCI&D include regular travel to provide care across the VA PIHCS catchment area.  The Nurse Practitioner SCI&D performs duties independently and reports to the SCI&D Medical Director with professional oversight by the Associate Director for Patient Care Services/Nurse Executive and the SCI&D Medical Director.  The position resides in the Primary Care Service line.

·         The Nurse Practitioner SCI&D is responsible for assessment, planning, coordinated provision, and evaluation of SCI&D-specific individual treatment plans for patients receiving care from the SCI&D Program of VA PIHCS.

·         The Nurse Practitioner SCI&D is responsible for leading the SCI&D team in provision of care to patients under his/her care, and is responsible for collaborating with services, both internal and external to the VA to facilitate care transition in order to effectively meet the patients’ needs.

·         The Nurse Practitioner SCI&D provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient’s goals.

·         The Nurse Practitioner SCI&D serves as an advocate for patients, team player to colleagues and he/she continues to enhance his/her own and the team’s professional growth, development and practice.
