Vacancy Identification Number (VIN): 871643 The Iowa City Veterans Affairs Health Care System (ICVAHCS) is recruiting for two full-time RN (Community Care Coordinator) in our Extended Care and Rehabilitation Department. The ICVAHCS is part of the Veterans Integrated Service Network (VISN) 23.

The Community Care Coordinator- Registered Nurse (RN) will be stationed at the Iowa City VA Health Care System, Iowa City, IA. The incumbent will provide coordination and planning of multiple health care services for veterans receiving health care services in the community. The incumbent will assure that necessary health care services are referred and received in accordance with Veterans Health Administration (VHA) Directives for purchased home health care and community nursing home oversight. In addition, the Care Coordinator provides on-going evaluation of care management services. The Community Care Coordinator works closely with Iowa City Veterans Administration Health Care System (IC VA HCS) primary care provider/s, professional team members, specialty clinics, internal or external services and community agencies. He/She is responsible for acting as a point of contact for home health care and community nursing home agencies for patients that receive their primary health care at the IC VA HCS and receive health care services in the community paid; for by the VA or any other means.

·         Citizen of the United States. 

·         Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). 

·         Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.  In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.    

·         Must be proficient in spoken and written English.

 Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.
