    Nursing Service, Hospital at Home (H@H) unit, at the Boise, Idaho VA Medical Center is now recruiting to fill a part-time Registered Nurse to work as a Home Care Nurse.  Work schedule is 8:00 am to 4:30 pm every other weekend (Saturday-Sunday) plus 4 hours during the week, each pay period (total 20 hours per 2 week pay period).

    Salary will be set by a Nursing Professional Standards Board and will be commensurate with experience and education. The SALARY RANGE indicated is the full-time amount. Less than full-time positions will be paid proportional to the number hours worked.

       The RN Home Care Nurse (Case Manager) is responsible for managing care across the health care continuum for a select group of clients. He/She provides professional assessment, coordination, telephone triage and planning of multiple health care services; acts on behalf of the veteran to assure that necessary clinical services are received and that progress is being made. The RN Case Manager is responsible to facilitate communication and collaboration between all members of the healthcare team involving the patient and family in the decision-making process; to minimize fragmentation of healthcare delivery; to optimize quality of patient and family outcomes in a cost-effective manner.  The incumbent works effectively with all levels of staff, to include collaborative efforts with other health professionals providing ongoing case management services.
       The RN demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice in providing care to patients with complex needs. Care will be provided in the least restrictive, most cost –effective manner through use of the clinical practice guidelines and clinical pathways. Evidence of health promotions and chronic disease indices are monitored through the clinical practice guidelines and performance measures. Demonstrates leadership by delivering and improving holistic care through collaborative strategies with others.
       The RN demonstrates leadership in the provision of age-specific care for veteran groups of varying ages, by recognizing the age related cognitive, physical, emotional, and chronological maturation needs of the adult and geriatric patient.  The RN case manager promotes independence; encourages self-care; involves family or designated caregiver with care planning, provides necessary safety measures and access to privacy; assists veteran to cope with changes affecting mobility, balance, cognition, psychosocial function, and physical/sensory changes based on physical, psychological and spiritual assessment.
       Receives and responds to skilled nursing care, and Palliative/Hospice Care consults within a timely manner and appropriately documents own assessments and interventions.  Participates in Interdisciplinary Team meetings in both acute and long-term care settings. Identifies potential Palliative/Hospice Care patients and Palliative/Hospice Care patient readmission to the facility.  Reviews options with patients and families, including costs, alternatives, risks/benefits and services.  Assists with the navigation of health care systems and intervenes or refers as needed to facilitate the achievement of patient outcomes.  Maintains privacy and confidentiality of all patient information, including electronic, print, and conversations.
