The RN Case Manager serves to coordinate patient care based on the nursing process and case management model.  The RN Case Manager demonstrates leadership in delivering and improving holistic care through collaborative strategies with others and adheres to established policies, standards of care and standards of practice.  Duties included, but are not limited to:    

 

 

1.      Demonstrates advanced clinical knowledge in assessing, planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care; recognizing the age related, cognitive, physical, emotional, and chronological maturation needs of the adult and geriatric patient.

2.      Demonstrates ability to collaborate with all levels of services and disciplines. Maintains current knowledge and skills based on evidence based practice.

3.      Advocates fiscal responsibility through monitoring of resources. Develop patient profiles to include high risk factors and high resource by computer tracking, analyze and develop interventions and strategies for reducing costs with appropriate recording and reporting. Utilizes creativity and innovation and recommends changes that will improve patient care or clinical efficiency.

4.      Triages and analyzes a collaborative team approach in identifying, analyzing, and resolving patient care problems.

5.      Provide patient/family education on prevention, wellness, maintenance and management of health issues (self-care issues), diagnostic tests and results, non-compliance to treatment regimen and make referrals to appropriate community agencies as needed.

6.      Provides age and population care and education. Promotes health promotion and risk reduction strategies.

7.      Enhances patient self management by providing information regarding all options, choices, and resources.

8.      Collects data base for research and participates in evidence based activities that are appropriate to the practice environment.

9.      Applies critical thinking and analytical skills to optimal patient care delivery.

10.  Serve as a link or bridge between the patient/family and the primary care providers, and the home health/hospice agencies and the providers.

Citizen of the United States. 

 

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). 

 

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.  In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.   

 

Must be proficient in spoken and written English.

 

Current certification in BLS

  

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.  The recommended salary may be at any point in the range listed for this vacancy.

Education:  

BSN or ADN/Diploma in Nursing with a bachelor's degree in a related filed is required.

 

Board certification from a VA-approved certifying body is preferred.
