The VA San Diego Healthcare System (VASDHS) in the La Jolla, CA main hospital has one (1) permanent full-time vacancy for a Registered Nurse, Care Coordinator OEF-OIF. 

 

The OEF-OIF care management program is built on a patient and family centered approach to care, and delivered by an interdisciplinary team of professionals with specialized knowledge in managing patients with complex care needs. This involves acting as the lead case manager for emerging medical, psychosocial, or rehabilitation problems, managing the continuum of care, care coordination, acting as patient and family advocates and assessing clinical outcomes and satisfaction. The Nurse Care Coordinator oversees the plan of care for those Veterans and active duty (referrals) with complex physical/medical conditions.

 

The Care Coordinator OEF-OIF Nurse works a full-time, 40 hours per week Monday through Friday schedule. Working overtime, evenings, weekends, and holidays are required to meet patient care needs.

 

BSN and a minimum of two years current experience in case management of complex traumatic injuries preferred.

 

Salary to be determined by the Nurse Professional Standard Board, based on education, experience and specialty qualifications.

The Nurse Care Coordinator OEF-OIF demonstrates performance and leadership that is broad enough to improve the care for a group of patients. The Nurse is responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, local officials, regional officials, or national officials. Program is defined as a substantial coordinated group of activities impacting patients and/or employees. Career path roles may include, but are not limited to clinical nurse leader, clinical nurse specialist, nurse coordinator, nurse educator, expert clinician, nurse manager, nurse practitioner, nurse recruiter, or researcher. The Nurse demonstrates (a) Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning; (b) Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes, (c) Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities, (d) Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise, (e) Guiding, developing, and supporting staff from a leadership perspective, and (f) Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.

Citizen of the United States.  Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).  Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.  In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Must be proficient in spoken and written English.

 

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.   Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research. Education/Experience Requirements: 

 

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience. 
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

PHYSICAL REQUIREMENTS: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to PTSD, TBI and/or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.
