VA Central Iowa Health Care System, primary duty site is Des Moines, IA. The VA Central Iowa Health Care System provides a full continuum of care between the Des Moines campus and our Community Based Outpatient Clinics (CBOCs) and is one of the Medical Centers that make up the VA Midwest Health Care Network (VISN 23). Employee selected may be expected to perform duties at other duty sites within VA Central Iowa Health Care System based on the needs of the organization.
The scope of practice of the Pharmacist covers direct and indirect patient care, drug therapy monitoring, prescriptive authority, quality assurance/improvement, staff/patient education and consultation. The pharmacist serves a multi-functional role and is expected to have an advanced knowledge of pharmacology, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.
Activities included in the daily function of the outpatient pharmacist include but are not limited to: Order entry, admission assessments, transfer assessments, identification and entry of adverse drug reactions into the system, review of patients on anticoagulation therapy, IV to oral conversions as listed by protocol, review of therapy and recommendations to providers as appropriate, and recommend renal dosing for therapy as appropriate. Clinical activities include but are not limited to: Provides oral and written consultation with other health care professionals regarding drug therapy selection or management in an accurate, timely and professional manner; Utilizes the highest level of clinical privileges for their position to manage assigned patients. Interfaces with providers, including physicians, PAs, and ARNPs, in the design of the veterans treatment regimen to achieve optimization of rational and cost-effective drug therapy; Identifies all drug-induced problems that may be affecting the patient or contributing to a need for hospitalization. This would include drug side effects, allergic reactions, toxicities, improper drug regimens, non-compliance, etc. Recommendations are made accordingly to the responsible provider either verbally or in writing; Provides and promotes cost effective pharmacy services, offering reasonable, appropriate alternatives to high cost drugs; and Provides care and/or services appropriate to the age of the patients being served. Assesses data reflective of the patients status and interpret the information needed to identify each patients requirements relative to their age-specific needs and to provide care needed as described in the services policies and procedures; Serves as a reference source and guide to proper IV therapy and scientific compounding techniques for physicians, nurses, and other members of the health care team.
