LPN will focus on clinical visits, exams and treatments with provider. The individual will be responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. Will work collaboratively with Core Team (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician ) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). Facility has implementation of Patient Aligned Care Teams (PACT) in areas consistent with Nursing Care Management and Coordination of Veterans. Clinical hours are Monday-Friday, 8-4:30.

- Possess current, full, active and unrestricted licensure.
- Pre-employment physical. Proficient in speaking and writing English language.
- Graduate of school of practical or vocational nursing approved by State agency and/or accredited by NLNAC.
GS-4 six months of qualifying experience at the GS-3 level or equivalent, fully meeting all performance requirements for the GS-3 LPN/LVN.
GS-5 one year of qualifying experience at the GS-4 level or equivalent, fully meeting all performance requirements for the GS-4 LPN/LVN.
GS-6 one year of qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.
