Tour of Duty:  midnight - 8:00am, Monday - Friday, with rotating weekends.

This position is located in the Inpatient Pharmacy Section of the Pharmacy Service at the Michael E. DeBakey VA Medical Center.   The primary functions will be the interpretation of healthcare providers' orders, dispensing of appropriate medications, and counseling patients on proper medication administration and storage.   The duties are performed in a complex tertiary care teaching medical center.  Interaction occurs with a large number of consultants, attending physicians and a variety of residency specialties.  

Duties include, but are not limited to: 

  • provide consultation to physicians and other healthcare professionals on basic information about drug therapy
  • intervene appropriately to increase effectiveness and minimize potential risks of drug therapy
  • evaluate patient or caregiver's ability to understand medication instruction and provide verbal and written counseling on his/her drug therapy
  • perform chart reviews, profile reviews, patient encounters, physician and nurse encounters, dosing service consults and other assigned clinical projects
  • evaluate physicians' orders as to appropriateness based on patient medication profiles
  • provide QC review and analysis for dispensing activities of pharmacy technicians
  • dispense investigation drugs following established Medical Center and VA policy
  • Other duties as assigned

BASIC REQUIREMENTS:  

U.S. citizenship.  Citizen of the United States.

Education.  Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.  Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).


Licensure.  Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

English Language Proficiency.  Pharmacists must be proficient in spoken and written English.

GRADE DETERMINATIONS:

In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates:

GS-11 Pharmacist

Experience, Education, and Licensure.  None beyond the basic requirements.

GS-12 Clinical Pharmacist (Full Performance Level)

Experience or Education.  In addition to the basic requirements, candidates must meet one of the following:

  • 1 year of experience equivalent to the next lower grade level, or
  • Completion of an ACPE-accredited Pharm.D. program.

Knowledge, Skills, and Abilities (KSAs).  Pharmacists assigned to this position must also demonstrate the following KSAs:

  1. Knowledge of professional pharmacy practice.
  2. Ability to communicate orally and in writing to both patients and health care staff.
  3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

PHYSICAL REQUIREMENTS:  Moderate lifting (15-44 lbs); moderate carrying (15-44 lbs); pushing (3 hrs); walking (3 hrs); standing (3 hrs); near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/50 and to 20/100 in the other; hearing (aid permitted); and mental and emotional stability.
