Duties:

 

The San Francisco has an intermittent (on-call) LVN position available.  The shift is 8:00 to 4:30 PM or 8:30 to 5:00 PM (no weekends, nights, evenings or holidays). This position will provide LVN Patient Aligned Care Team (PACT) staffing coverage for the San Francisco VA Downtown and San Bruno Community Based Outpatient Clinics (CBOCs).

These clinics service approximately 2000-3000 patients in primary care. The clinics also provide some specialty care services.  Become a member of this health care team providing compassionate healthcare to veterans who follows the Patient Aligned Care Team (PACT) model of care delivery. We are looking for an energetic, flexible and motivated LVN with excellent Primary Care skills to contribute to the growth and progress of the PACT -oriented delivery of health care that we provide for our nation's veterans. 

Duties include but are not limited to: patient education, completion of clinical reminders, injections, phlebotomy, assisting with wound care, rooming patients, completion of vital signs and other assignments related to PACT.  This involves working in active collaboration and communication with an assigned Core Team (patient, primary care provider, RN, clerk) and Expanded Team (family/caregiver, internal and community based services) in order to effectively meet the patients’ needs.

The LVN will need to be able to prioritize and complete nursing care to facilitate timely patient flow, have excellent interpersonal communication skills and be a team player.

 

One full year of salaried LVN experience in Ambulatory Care, ED, telehealth, triage, or an urgent care setting is required.

The LVN will need to be able to follow and comply with infection control policy, prioritize and complete nursing care to facilitate timely patient flow in the specialty clinics and primary care, have excellent interpersonal communication, organizational skills and be a team player. Primary Care, Dermatology, Eye clinic, podiatry, and inpatient care experience is a plus for this nursing position.

 

Schedule/Hours: 

Monday to  Friday, 8:00 – 4:30pm or 8:30 – 5:00pm.  Hours are not guaranteed. All intermittent (on-call) nurses are required to be available a minimum of four shifts per month.

LICENSURE: Must possess a current, full, active and unrestricted registration as a practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
