The Cancer Care Coordinator's dedicated responsibility is the coordination of the cancer patient's journey through the care system, at times across disciplines and care settings.  Provide patient centered service that will improve patient safety, satisfaction, and facilitate quality and timeless care.

 

Duties:

 

The RNP ensures compliance with all policies, procedures and protocols as well as having clinical, therapeutic and administrative duties and responsibilities not limited to:

 

Coordinates medical oncology and multidisciplinary staff in the delivery of complex outpatient and inpatient care including patient evaluation, performing or arranging for diagnostic and therapeutic procedures.  Schedule patients for admissions, chemotherapy treatment, and gathering baseline and follow up data and teaching patients and family as needed.  Makes appropriate assessments and referrals to meet the unique needs of this specialized patient population and facilitates consultations and participates in the care of patients seen in oncology clinics and provides follow up care.  Participates in the Cancer Care Committee, Tumor Boards and a major role in the multidisciplinary patients conferences.  Facilitates patient's participation in ongoing clinical research through collaboration with study coordinators and research nurses.  Also, initiates and implementing solving strategies for patients with complex care plans and develops and utilizes practice guidelines or clinical pathways as necessary to streamline multi-disciplinary evaluation and treatment for cancer patients.  Participates in quality improvement activities, including the monitoring of treatment guidelines.

Citizen of the United States.  Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).  Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.  In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.    Must be proficient in spoken and written English.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.   Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research.    

Education/Experience Requirements: 

  • Masters degree in nursing required.
  • License and ANCC certification as NP required.
  • 3 years of clinical experience as a registered nurse preferred.

Additional requirement for this position.

  • Must have one (1) year full-time RN nursing experience.
  • BLS/ACLS required

 

The following requirements are preferred for this position.

·        Two (2) years of recent RNP experience  in evaluation and management of complex illnesses preferred.
