Work Schedule: 

The position is full-time, Monday through Friday 8:00am – 4:30pm.

 

Duty Location: 

Green Bay, WI

 

The duty station of the position is in Green Bay, Wisconsin; however, this position may require regular and recurring travel to the Milwaukee VA Medical Center and CBOCs in Appleton, Cleveland, and Union Grove, Wisconsin.

 

The Green Bay Outpatient Clinic official hours of operation are Monday through Friday, 8am – 4:30pm; however, during the initial orientation period, scheduled work days and tour of duty may vary.

 

The Initial orientation period may require daily/weekly travel to the Milwaukee VA Medical Center for several months.  The length of the orientation period will be dependent upon the selectee’s experience and progression.  The selectee may be required to sign an orientation/travel agreement.

 

Duties:

The primary function of this Pharmacist position is to provide pharmaceutical care services to patients in primary care clinics. The Pharmacist possesses a professional knowledge of drug information, pharmacology, pharmacokinetics, and clinical pharmacy practice. The Pharmacist functions with other health care professionals as an essential component of the multidisciplinary team. He/She serves as a resource concerning complex pharmaceutical treatment, which affects patient care.

·         Functions administratively under the Primary Care Team Managers/Primary Care Division. The incumbent functions clinically under the Professional Practice Pharmacist Consultant.

·         Interfaces with the Primary Care Physicians and/or various other health care providers in primary care in the design of the veteran's treatment regimen.

·         Serves as an authoritative information source on drugs, including investigational drugs, and their utilization in therapy.

·         Evaluates the drug literature by analysis of experimental design and evaluation in order to compare and contrast therapeutic regimens and roles for new drugs.

·         Aids in selection of appropriate dosage regimens from current pharmacokinetic principles and specific patient parameters.  Provides follow-up when sub/supra-therapeutic drug levels are obtained for primary care patients.

·         Initiates therapy as defined by individual scope of practice statements.

·         Interviews and examines patients, reviews laboratory and diagnostic test results, and reviews medication profiles to identify any medication related problems that may be affecting the patient or contributing to the need for hospitalization.

·         Provides medication renewals to patients who will exhaust the supply of their medication(s) prior to their next scheduled clinic appointment.

·         Monitors patients' therapeutic outcomes using physical exam, medical history, chart review, and laboratory/diagnostic data as defined in the scope of practice statements.

·         Educates patients, family members, and/or caregivers on the proper use and storage of medications through daily interactions.

·         Prepares bulletins, newsletters and policy memos to inform physicians, nurses, and other pertinent staff on the appropriate utilization of medications as requested.

BASIC REQUIREMENTS:

·         Must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

·         Must  possess a current, full, active, and unrestricted license to practice pharmacy in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia.

·         Must be proficient in both spoken and written English as required by 38 U.S.C. 7402 (d), and 7407 (d).

·         Must be a U.S. Citizen.

 

GRADE REQUIREMENTS:

In addition to the Basic Requirements you must meet the following GS-13 grade requirements:

Must have the equivalent of 1 year of creditable experience at the GS-12 level or equivalent, which is directly related to the Clinical Pharmacy Specialist position that is to be filled and have demonstrated the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions, Expert understanding of regulatory and quality standards in the Primary Care program area, Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in the Primary Care program area or area of clinical expertise, Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of Primary Care, and Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

OR

Must have completed a 1 year post-Pharm.D. ASHP accredited Residency

OR

Must be completing a 1 year post-Pharm.D. ASHP accredited VA Residency by July 2013

Combining Education and Experience: Combinations of successfully completed post-high school education and experience may be used to meet total qualification requirements. It will be computed by first determining the applicant's total qualifying experience as a percentage of the experience required for the grade level; then determining the applicant's education as a percentage of the education required for the grade level; and then adding the two percentages. The total percentages must equal at least 100 percent to qualify an applicant for that grade level.
