The Durham VA Medical Center is recruiting for two (2) Medical Records Technicians to be assigned to the Health Information Management Section (HIMS) of Health Administration Service.

The nature of the work involves the full scope of review, analysis, problem identification and resolution, evaluations, coordination, monitoring and follow-up in the health information-utilization management program of the Medical Center. The incumbent is expected to anticipate needs and to design and maintain systems that will function through the various house staff and attending physician rotations. Analytical ability, judgment, discretion, and a high degree of tact and negotiating ability are required on a daily basis to carry out the functions of the position.

The incumbent is responsible for; (1) providing administrative support for the Medical Center Health Information Program, (2) abstraction of clinical information for ongoing regulatory reviews, (3) advising the clinical treatment team on issues related to record completion and documentation requirements, and (4) assisting with processing legal requests for records and/or testimony from VA Regional Counsel and third party lawyers. The incumbent is responsible for the following functions:

Contacts physicians and other ancillary service personnel regarding dictation and/or signatures. Assists medical staff by providing information as to required documentation. Communicates very closely with the medical staff to answer questions and resolve problems to insure quality documentation in the medical records.

Prepares lists of records needed for signature(s) that are located in the incomplete record areas for distribution to appropriate service lines (i.e., Medicine, Surgery, Psychiatry, etc).

Monitors delinquent dictation list for follow-up purposes on necessary documents (i.e., operation reports, discharge summaries, etc.) that are needed for completion of medical records.

Notifies appropriate physician and other personnel of the status of dictation.

Monitors house staff rotation schedule and contacts resident house staff to insure completion of records prior to departure from this Medical Center.

Utilizes basic knowledge of medical terminology, anatomy and physiology, and ICD-9-CM coding to research and collect information for various medical studies and research projects utilizing the Patient Treatment File (PTF) and PTF microfiche.

Assists with release of information from patients' records when expertise is needed, or when backlog warrants.

Conducts periodic audits of records processing activities.

Participate in approved quality improvement focused reviews.

Coordinates review activities with the Quality Improvement Program to satisfy external accreditation requirements, and to satisfy current Medical Center performance measures.

Assists with capture and reporting of documentation deficiencies as needed including downloading data and creating trended reports.

BASIC REQUIREMENTS

a. Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with chapter 3, section A, paragraph 3g, this part.)

b. Experience and Education

(1) Experience. Two years of experience that demonstrates the applicant’s ability to perform the work, or provides familiarity with the work. Six months of the required 2 years of experience must have provided the knowledge, skills, abilities, and other characteristics (KSAs) needed to perform MRT work.

OR,

(2) Education. Successful completion of an associate’s degree with a major field of study in medical record technology/health information technology which was accredited by the American Health Information Management Association (AHIMA) at the time the program was completed.

OR,

(3) Experience/Education Combination. Equivalent combinations of experience and education are qualifying. The following educational/training substitutions are appropriate in this provision for combining education and experience:

(a) Successful completion of an associate’s degree in a field of study other than medical records/health information will substitute for 18 months of the required experience.

(b) Successful completion of a course for medical technicians, hospital corpsmen, medical service specialists, or hospital training, obtained in a training program given by the Armed Forces or the U.S. Maritime Service under close medical and professional supervision, may be substituted on a month-for-month basis for up to 1 year of experience.VA HANDBOOK 5005/15 MARCH 17, 2006 PART II APPENDIX G35 II-G35-2

(c) Successful completion of a post-high school course of study in medical record technology, which includes courses in anatomy, physiology, and medical record techniques and procedures, may substitute on a month-for-month basis for up to 1 year of experience.

(d) Successful completion of post-high school courses for medical administrative personnel, obtained in a closely supervised training program given by the Armed Forces, may be substituted for experience on a month-for-month basis for up to 6 months provided the training program included courses in anatomy, physiology, and medical record techniques and procedures.
