The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking an experienced Nurse Practitioner (NP) to work in our Home-Based Primary Care (HBPC) Department. The NP will provide home-based primary care to a panel of Veteran patients in a patient-centered, healthcare team approach. The NP will be responsible of provide direct and indirect patient care that includes assessment, diagnosis, planning, and treatment of health problems for Veterans in the Los Angeles area. The HBPC NP is a midlevel practitioner who provides primary care in the home to a clinically and psychosocially complex population of chronically ill or severely disabled veterans. Duties include assessment, planning, coordinated provision, and evaluation of individual treatment plans for patients receiving care from the Home Based Primary Care (HBPC) program. The NP is responsible for leading the interdisciplinary HBPC team in provision of care to their individual caseload of patients. He/she collaborates with a lead physician designee in the team assigned and is accountable for outcomes of his/her practice. The HBPC NP is accountable to the HBPC Program Director and reports directly to the Geriatrics and Rehabilitative Care (GRC) Lead NP. This position is not open to novice Nurse Practitioners.
Monday-Friday Day Shift
