TEXAS ARMY NATIONAL GUARD

 

EXCEPTED TECHNICIAN VACANCY ANNOUNCEMENT

 

This position is located in the Joint Forces Headquarters, Medical Detachment, Camp Mabry, Austin, TX.  The purpose of this position is to  serve as a Clinical Nurse Case Manager.  Uses a collaborative team approach  to assess, plan, implement, coordinate, monitor, and evaluate  options and services to meet complex  and routine healthcare  needs of the National Guard population; promoting timely, efficient  and effective  care delivery.  Uses facilitation, negotiation, advocacy, communication, education, and research to support   timely, comprehensive, patient  centered, quality healthcare  delivery.    The primary   responsibility of the position  is to facilitate care of soldiers with  regard to the delivery  of case management services as an advocate   member  of an interdisciplinary team; to facilitate movement between appropriate levels of care, including  seamless transition  from inpatient to ambulatory phases, ensuring health needs are met, and soldier readiness for release from   active duty (REFRAD) or Physical Disability  Evaluation  System (PDES) referral   through effective   communication, collaboration, knowledge  of available  resources, and nursing processes to promote   quality outcomes  across the health  care continuum. Supports  soldiers and their  family  members in the coordination of medical and social resources focusing on assessing, planning, implementing, coordinating, monitoring, and evaluating  options  and services to meet fitness for duty   evaluation, health  care goals and needs.

As a Nurse (Clinical/Case Management) with the Texas Army National Guard (ARNG), your duties will include, but will not be limited to:

    • 1. Serves as Nurse Case Manager
    • a. Functions as a case manager  and/or  team leader for mobilized  National Guard members, as well as those awaiting  Medical Evaluation  Board and those returning to duty that require  management of health care services.   Responsible  for intake and assessment of these personnel presenting  telephonically  or in-person, while awaiting  a Medical Evaluation  Board, REFRAD, or return  to duty.  Through effective  screening  and assessment, interdisciplinary collaboration, determines the prioritization, goals, and care plan for health  and psycho-social  needs. Advocates and coordinates  healthcare  delivery  to facilitate fitness for duty  evaluation.  Maintains documentation and database  for outcome evaluation  as required.  Prepares regular, periodic, and ad-hoc reports  for Case Management.
    • b. Based upon triaging, comprehensive intake, and ongoing  monitoring, the case manager  facilitates  entry into the appropriate  level of care, at the appropriate level of clinical urgency and within  TRICARE access standards.   Interfaces with the soldier, family members, Command and Staff personnel, primary  care providers, TRICARE contract personnel, Warrior Transition Battalion, Veterans Administration, other military treatment facilities, assorted agencies, and TRICARE network  providers/facilities as appropriate in order to effectively  arrange  soldier healthcare  services.
    • c. Facilitates  communication and coordinates  medical services with other  members  of the health care team and medical facilities thus minimizing fragmentation in the delivery  of services.   Facilitates  needed referrals  for other requisite  specialty  services within  the direct and purchased  care sectors.
    • d. Works in close and effective  collaboration with physicians  and other healthcare and military personnel in independently performing difficult and complex, assessment, review, and analysis of cases with  an established  timeframe after  identification for intake. Interviews, counsels, collects data, and performs  a comprehensive nursing assessment, following  nursing process and case management standards/guidelines, utilizing  a holistic approach to identify  existing  and potential  problems  with the goal of creating  a functional individualized plan of care for soldiers to include their  family  members, to optimize current health/psychosocial status and benefits of case management services, and to facilitate  their fitness for duty evaluation  or referral  to PDES. Coordinates  with PCM and interdisciplinary healthcare  team to develop time line protocols  which delineate  the expected process of care delivery  and monitors  progress, exceptions, deviations, and differences  to identify  and anticipate problems  and reports  deviations  from the expected care plan to the PCM for direction.
    • e. Participates  in case management and joint  planning  with Health Care Providers, Health Benefits Advisors, Health Care Finders, and Utilization Review officials in military and civilian  health care facilities  to identify beneficiaries  and beneficiary  families with long-term health care needs and/or  high risk, and high cost diagnoses.   Develops treatment plans with the primary  care provider, Medical Evaluation  Board (MEB)Physician, other health  professionals, family  and soldier to meet  individual  soldier needs and coordinates for delivery  of such services.   Helps identify  appropriate provider  and facilities  throughout the continuum of services while ensuring  that  available  resources are being used in a timely  and cost effective  manner.   Introduces innovative nursing techniques, practices, and approaches  in collaboration  with  primary care provider, and other health care providers  to provide  quality, efficient, comprehensive, and cost effective services throughout the continuum  of care.   Monitors  medications  to include routine  medication  review and education  within  specified time  frames, care plan, and clinical appointment  schedule adherences. Documents/updates assessment, goals, and care plan.  Obtains regulations, policy manuals, guidelines, standards, and handouts  from other agencies and develops  procedural  guidelines  to provide  access to these programs  in concert  with TRICARE. Develops guidelines  for interfacing  with  interdisciplinary federal agencies/services, their  civilian  healthcare  delivery  system supporting TRICARE counterparts, ensures their compliance  with case management/ Warrior Transition  Unit Programs goals, standards  of practice, and awareness of these guidelines.
    • f. Participates  as an active  member  of the Case Management team. This includes serving as a point  of contact  between selected soldiers and the treatment team, triaging  and answering  treatment questions and appropriately referring questions to the primary  clinician, when indicated.  Provides appropriate educational  information to soldiers and maintains  clinic resources  for social and health services in the community (e.g. literature, web-based resources, professional  organization  information, medication specific education materials, self-help  groups, support  groups, etc). TRICARE benefits, cost factors or other issues that may affect the health care team's  or the soldier's treatment decisions. Facilitates interdisciplinary treatment planning  conferences and team meetings  in conjunction with the assigned resident  for the team.
    • g. Develops, in collaboration with nursing  leadership,   outcome  instruments to be used to measure clinical productivity.  Develops operating  procedures, such as clinical pathways, for Warrior Soldier population  within the case management program, promotes evidenced base practice and evaluates outcomes and effectiveness.   Identifies problems  with health care access/utilization in both direct and purchase care sector and recommends alternatives to overcome  these difficulties, e.g. admissions, transition to outpatient services, equipment and services, soldier issues/inquiries, as well as, quality/process improvements.  Identifies strategies  to improve  soldier access and satisfiers, administrative burdens, and cost effectiveness  for the healthcare  delivery system  which evolve from metric  and problem  analysis, trending, and reporting mechanisms.   Provides guidance on appointment access protocols  to match population/soldier's needs with available  resources in a timely  manner.   Responsible and accountable  for assessing the specific, as well as, overall  usefulness/effectiveness of case management services. Develops innovative soldier and family education modules regarding disease process; diagnostic  procedures; use of diagnostic  manuals related  to disease and the standards  of care; expected and possible reactions  to medication; medication compliance; health  maintenance  and relapse prevention using a variety  of multimedia resources.
    • 2. Performs Nurse Educator Functions
    • a. Coordinates, implements, and evaluates the efficacy of the individualized treatment plans for selected high cost/high risk soldiers.  This evaluation  process involves continuous monitoring and analysis of the soldier's response to the services provided  and timely  interventions/modifications when variances  occur.  Identifies and manages emergencies  ensuring soldier safety, appropriate and timely  notification of primary  care manager.
    • b. Provides counseling and continued  consultative support  to soldiers and their  families regarding disease process, rehabilitative needs and health care management.   Provides supportive therapy, counseling  and guidance, and health  teaching  to soldiers and families through individual  or group conferences designed to assist the soldier in understanding, accepting  and following  medical  recommendations.  Facilitates  referral for professional services directed at restoring  the soldier to optimum psychosocial and health adjustment within  the soldier's capabilities  when indicated.   Utilizes community resources to assist the soldier to resume life in the community or to learn to live within  limits  of disabilities.
    • c. Oversees daily interactions with assigned soldiers; telephone  triage;  referrals  made to other services within  the hospital and liaison between referral  sources and the program, documentation and chart maintenance.  Assists and provides  direction  with clinical decision-making.  Coordinates in-service  training for staff members  to include Performance Improvement Projects and development of a Soldier Acuity Tool.  Illustrates program  routine  and expectations according  to current  guidelines, SOPs, and Joint Commission on Accreditation of Healthcare  Organizations  (JCAHO) Standards. Collaborates  with clinical nurses and other members  of the health  care team to problem solve complex clinical situations  for special system  needs.

General Experience Required 

Candidates at all grades must have active, current registration as a professional nurse and meet one of the following requirements:

 

a.   Graduation with a bachelor's or higher degree in nursing.

 

                                           -- or--

 

b.   Graduation from a 3-year (at least 30 months) diploma program of professional nursing.

 

                                           -- or--

 

c.     Graduation from an associate degree program or other program of at least 2 years in a school of professional nursing.

 

                                           --and--

 

Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.

10. Ability to work in close and effective collaboration with physicians and other healthcare and military personnel in independently performing difficult and complex, assessment, review, and analysis of cases with and established timeframe after identification for intake.

11. Ability to develop treatment plans with the primary care provider, Medical Evaluation Board (MEB) Physician, other health professionals, family and Soldier to meet individual Soldier needs and coordinates for delivery of such services.
