Position Summary:

The SCAN-ECHO/E-Consult APN is a clinical position in the Renal/Nephrology Section with administrative responsibility for planning SCAN-ECHO/E-Consult sessions and coordinating the development and implementation of local SCAN-ECHO/E-Consult conferences.   The SCAN-ECHO/E-Consult APN is responsible for participating in the development and implementation of the quality, safety, efficiency, and satisfaction outcomes assessment.   In addition the APN is responsible for assuring the acquisition of continuing education credit for SCAN-ECHO/E-Consult participants.

SCAN-ECHO is training through the VISN (Veterans Integrated Service Network) on Live Meeting for Primary Care providers (PCPs) by Subspecialty Experts for treatment of their patients. Different formulas for making interactive case discussions between PCPs and our subspecialty group into SCAN-ECHO presentations are being utilized.  One involves a case study that the PCP presents via Tandberg connection or locally and then one or more of the Subspecialty group discuss the best way to treat this patient and diagnosis so that all VISN providers can learn about this condition.

E-Consults are requests entered into the electronic clinical record either for a rapid written or phone response to a focused question, a Tandberg-to-Tandberg interactive long distance visit or a traditional face-to-face appointment at our site. This patient care activity may lead to a SCAN-ECHO session between the referring PCP and one of more of our group.

The overall goal is to use advanced communication and imaging technology to facilitate interaction and education between our subspecialty group and VISN providers and to provide more accessible and convenient subspecialty care for our Veteran patients across a huge geographic area.

The SCAN-ECHO/E-Consult APN is responsible for contributing to the ongoing development and evaluation of the national SCAN-ECHO/E-Consult program by data collection and feedback and contributing to refinement of policy and procedure, including standard operation procedures.  The SCAN-ECHO/E-Consult APN is responsible for assisting the national program office in outreach as a conduit of communication between local health care teams and leadership.  The SCAN-ECHO/E-Consult APN is expected to contribute to the national program effort by providing consultation to and replication of the program in newly developing sites.  The position promotes safe, effective patient-centered and timely specialty care to patients.

This position requires the ability to assist in forming and promoting relationships between primary care and subspecialty care, to collaborate with local, VISN and national leadership and to promote the vision of the SCAN/E-Consult program.

This position requires a mixture of direct patient care, program adminstration and coordination and leadership skill.

The SCAN-ECHO/E-Consult APN will also maintain a subspecialty practice in Renal/Nephrology diseases by participating in the full range of clinical activities of the Renal/Nephrology Section of the Denver VA Medical Center.  These activities include continuity outpatient clinics, tele-health clinics, scheduling and coordination of renal procedures and assisting in other patient care activities as determine by Chief, Renal/Nephrology Section.  This AP"N will closely with several subspecialty physicians each with complementary areas of expertise.

Hours:  Monday – Friday, 7:30am – 4pm

Candidate must be willing to work weekends ,irregular work hours and extended hours .  The candidate must be willing to be placed, detailed or temporarily assigned to other related services and/or locations, if necessary.

Qualifications:

(1)   Citizen or national of the United States

(2)   English Language Proficiency:   If you are appointed to a direct patient care position, you must be proficient in spoken and written English as required by 38 U.S.C. 7402 (d) and 7407 (d).

(3)   Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia.

(4)  Graduate of a school of professional nursing approved by the appropriate State agency and accredited by the National League of Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).

(5) Master's degree in nursing (Adult or Family Nurse Practitioner program in the specialty area) from a NLNAC or CCNE accredited program required. 

(6) National certification as NP by the American Nurses Association, SNCC, AANP or ANCC (American Nurses Credentialing Center) is required.

(7) Listed on Advanced Practice Registry authorizing use of NP designation.  

(8) Prescriptive Authority.

(9) Two – three years of relevant experience.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.   Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research.    

Education/Experience Requirements: 

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience. 
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
