The Clinical Pharmacist Specialist-Inpatient has the principal duties in providing highly innovate and progressive clinical pharmacy services and developing and directing clinicial pharmacy programs in the Inpatient Care Area.  Clinical pharmacy duties are also performed in other areas of the Medical Center as necessity requires and time permits. He/she is responsible for providing pharmacist and technician staff development through education and training programs.  Plans, coordinates, and conducts education and training programs which will provide the staff with competencies to provide total pharmaceutical care.  The incumbent provides and coordinates educational programs for patients and for medical and nusring staff.  Additional general duties are:

    a). Shares responsibility with the other clinical pharmacists for coordinating and directing drug information and practice activities and acting as a PRIME residency preceptor.  Develops therapeutic drug policy through medical literature evaluation and the application of pharmacoeconomic principles and methods.

    b). Assures the safe, appropriate, cost-effective use of drugs in assigned patient care areas through the application of specialized knowledge and the use of judgement in the colleciton and interpretation of data specific to each patient and in tactful interactions with other professionals.

    c). Active participant in the Pharmacy Service Quality Improvement program.

    d). Maintains a practice site in an Inpatient Care Area and serves as a clinical resource for paitent nutrition support.

    e). Shares the responsibility for cost containment by recommending to physicians the most efficacious and cost effective therapy.  Prepares reports documenting cost containment for the P&T committee when needed and/or requested.

    f). Actively participates in Pharmacy and Therapeutics (P&T) Committee activities and shares responsibility for Drug Use Evaluation and Adverse Drug Reaction Reporting with appropriate health care professionals.

`The Clinical Pharmacist-Outpatient in this assignment handles routine medication related activities in accordance with local, Veteran Integrated Service Network (VISN), and National policies and regulations.  These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconcilliation; providing drug information; assisting in formulary management including therapeutic substitutions, non-formulary reviews and medication usage evaluations; documenting and assessing adverse drug events; assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distrlibution.

BASIC REQUIREMENTS:  

  • U.S. citizenship.  
  • Graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP) OR Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia.
  • Must be able to lift/carry pharmaceutical supplies weighing at least 50 lbs., bending and stooping, and regular and recurring physical exertion involving standing for prolonged periods of time.
  • Must be proficient in spoken and written English.

 

GRADE DETERMINATIONS:

GS-12 Level: To qualify at the GS-12 level you must have completed the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled OR completed a 1 year post-Pharm.D. ASHP accredited Residency.

 

In addition to the basic requirements above, applicants appointed at the grade level of GS-12 are expected to possess and provide verification of the following knowledge, skills, and abilities;

 

  • Comprehensive knowledge of a specialized area of clinical pharmacy practice or speciality area of pharmacy such as quality assurance/utilization review, informatics systsms, drug information, etc;
  • Comprehensive knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, pharmacoeconomics and pharmacotherapeutics;
  • Skill in monitoring and assessing the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters;
  • Knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to health care;
  • Ability to communicate orally to persuade and influence clinical decisions;
  • Ability to communicate in writing to persuade and influence clinical decisions;
  • Comprehensive knowledge of DEA, FDA, VA, the State (Territory of District of Columbia, if appropriate), TJC and other standards as related to the distribution and control of scheduled and non-scheduled drugs to inpatients.
