The Patient Aligned Care Team (PACT) is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion, coordination and chronic disease management. The social worker in this Veteran-centric approach often functions as a care coordinator with a panel of veterans to ensure that health care meets the needs, as defined by the Veteran. Social Workers are uniquely prepared for this role by virtue of their education, training, and experience which prepares them to understand the person-in-situation and the influences that family and community have in impacting the health-care needs of Veterans.

Preferred Requirement:  LCSW or LISW

Work Schedule:  Full-Time, 40 hours per week

BASIC REQUIREMENTS:

U.S. Citizenship - Be a citizen of the United States

Physical Standards - Applicants must meet physical standards for the position. A physical examination prior to placement is required.

English Language Proficiency - Individuals appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)

Education - Possess a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)

Licensure - Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master’s degree level. Current state requirements may be found on the OHRM website.

Exceptions - Applicants who otherwise meet the minimum requirements in the basic qualification standard except for licensure, certification or registration may be given a temporary appointment subject to obtaining State requirements. Those who fail to obtain State licensure, certification or registration after 3 years must be removed from the GS-185 Social Worker series, which may result in termination of employment. A statement citing this condition of employment must be signed by individuals appointed under these provisions. This exception only applies to up to the full performance level GS-11

GRADE DETERMINATIONS:

GS-09:  this is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-09 level are working toward completion of prerequisites for licensure or certification.

GS-11:  promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, in addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.

GRADE REQUIREMENTS:  The grade of the position is dependent upon the applicant’s qualifications and will be determined by a Professional Standards Board
