The Fargo VA Health Care System is a Level 2 Medical Center and a member of Veterans Integrated Service Network 23. This facility combines a 39 bed medical facility along with a 38 bed Community Living Center providing transitional care, both accredited by the Joint Commission on Accreditation of Healthcare Organizations. Fargo also has a Department of Veterans Affairs Benefits Office on campus, offering veterans the unique opportunity to obtain health care, benefits services, and Readjustment Counseling at one location. Due to the growing veteran population in our service area, nine Community Based Outpatient Clinics have been established with the tenth planned for opening in late 2012. The Fargo Health Care System provides a wide spectrum of primary and tertiary acute and extended care inpatient and outpatient activities in an academic setting. The Fargo Health Care System has an active research program and has a successful affiliation with the University of North Dakota School of Medicine.

 

NOTE:  The Professional Standards Board (a peer-review group) will review the selected candidate’s education and experience and recommend the step at which the candidate will be placed in that specific grade.  The salary is based on the grade and step approved for the selected candidate.

The clinical pharmacy specialist is primarily responsible for providing pharmaceutical services to adult patients of all ages specifically in the Surgical and Specialty Care. The Clinical Pharmacy Specialist spends greater than 50% of his/her time in direct patient care activities.   Services are provided in accordance with approved policies and procedures and accepted pharmacy practice. Responsibilities include interpreting medication orders written by providers; dispensing appropriate medications as necessary; counseling patients on proper medication administration and storage; evaluating patients' medication therapy; performing medication reconciliation tasks according to guidelines and policy; attending surgical bedside rounds; providing recommendations to providers; participating in Antimicrobial Stewardship Program (ASP); providing education to staff and patients; conducting research as requested or desired; providing monitoring and dosing services for pharmacokinetics, erythropoiesis stimulating agents (ESAs),anticoagulation, and Total Parenteral Nutrition (TPN); and participating as an active member on committees as requested.   The Clinical Pharmacy Specialist may also manage medication programs along with developing medication policies and procedures as requested/required.    Tour of Duty is primarily days with rotations on weekends and holidays.

 

The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients.  A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services.  This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.  Pharmacists assigned to this position must demonstrate the following KSAs:

 

     a.   Ability to communicate orally and in writing to persuade and influence clinical and management decisions.


     b.   Expert understanding of regulatory and quality standards for their program area.

 

     c.   Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

 

     d.   Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

 

     e.   Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

BASIC REQUIREMENTS:  

  • U.S. citizenship. 
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy 9BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.  Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education website at Http://www.acpe-accredit.org/ 
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the U.S. or the District of Columbia;
  • Proficiency in spoken and written English and 
  • Must meet the physical requirements of the position. 

GRADE DETERMINATIONS:

Specialized Experience GS-13 requires: In addition to meeting all basic requirements, applicants must have one year of specialized experience as a Clinical Pharmacy Specialist at the GS-12 level in the Federal Service or equivalent; or board certification recognized by the American Pharmaceutical Associations Board of Pharmaceutical Specialties, or the American Society of Consultant Pharmacists (or equivalent) or completion of two year post-Pharm.D. ASHP fellowship in a clinical or specialized program.  Specialized experience includes: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
