 The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking an experienced Nurse Practitioner (NP) to work in our Primary and Ambulatory Outpatient Care Clinic. The NP will provide primary care to a panel of Veteran patients in a patient-centered, healthcare team approach.  He/she will function as a member of the Patient Aligned Care Teams (PACT) with emphasis on improving health outcomes through a system of care management and coordination. He/she collaborates with a lead physician designee in the team assigned. The NP is accountable to the Associate Chief, Nursing Service Community/Primary & Ambulatory Care and will report directly to the Lead Nurse Practitioner. This position is not open to novice Nurse Practitioners.

 

Relocation expenses will not be reimbursed.

1. Evaluates the veteran’s physical health, family and psychosocial history to identify health care needs and documents in medical record.

2.    Provides primary and continuity of care to veterans.

3.    Performs physical examinations using   techniques such as inspection, palpation, and auscultation.

4.    Maintains expected work load panel of patients, assuring their continuity of care and that health maintenance and performance measures are achieved.

5.    Provides health education to Veterans

6.    Uses CPRS to document patient physical exam, treatment and plan for care. 

7.    Completes all relevant clinical performance reminders and participates in quality improvement activities and peer review.

8.    Assists with the design and implementation of delivery systems/ programs to facilitate the care of high risk veterans.

9.    Collaborates with team members to provide continuity of clinical care and implement of treatment plan.

10. Orders and interprets diagnostic tests.

11. Prescribes medications in accordance with scope of practice.

12. Participates in patient, staff and community education.

13. Participates in medical and/or nursing research and quality improvement.


QUALIFICATIONS REQUIRED:

1.    A Master’s Degree in Nursing (MSN) or Doctorate of Nursing (DNP) required.

2.    A graduate of a school of professional nursing approved by the appropriate State agency and accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirements of graduation from an approved school of professional nursing.

3.    Current Furnishing License is required

4.    Current DEA certificate for controlled substances schedules II-V is required.

5.    National Certification as a Nurse Practitioner is required.

6.    Basic Life Support (BLS) certification from the American Heart Association, American Red Cross, or Military Training Network

7.    Full, active, current and unrestricted registration as a Registered Nurse Practitioner in a State, Territory, or Commonwealth of the U.S. or in the District of Columbia.

8.    Computer literacy proficiency with in-dept experience with the use of Microsoft Office programs or comparable word processing, spreadsheet and graphic software and the ability to learn new programs specific to the VA.

9.    Minimum (3) three years clinical registered nursing practice.

10. Minimum (1) one year previous NP experience in an Ambulatory or Primary Care setting

11. Knowledge and experience with comprehensive Diabetes Management

12. Experience working with veterans and/ or homeless populations preferred
