The Michael E. DeBakey VA Medical Center is seeking a Registered Dietitian to be a member of the Home Based Primary Care team.  The Dietitian will perform the initial and ongoing assessments of the patient’s nutritional status, recommend special diets and dietary modifications, monitor for food and drug interactions, and contribute to optimal disease management by educating the Veteran, caregiver, and staff.

The Clinical Dietitian is the medical nutrition expert responsible for planning, developing, and directing nutritional care activities for all patients, including those with complex medical and nutritional needs.  He/she supports the facility’s mission and coordinates nutritional care with other health care providers, the patient, and the patient’s family.  The increasing complexity and specialization in nutrition requires that the clinical dietitian have expertise in a wide range of disease entities, treatment modalities and drug/nutrient interactions.  The incumbent serves as the Home Based Primary Care (HBPC) Nutrition Specialist Dietitian.  The specialist is a professional member of the interdisciplinary HBPC Team.  The specialist is considered the medical nutrition expert for HBPC and is responsible for the medical nutrition treatment of veteran beneficiaries on the HBPC Program.  The dietitian is also responsible for providing professional guidance and education in the medical nutrition care of HBPC patients for medical residents, nursing, allied health, and dietetic students.

The work requires a valid driver's license and the ability to drive long distances on sub standard roads and access homes that may be in poor condition and have poor environmental controls.  The primary physical demand is fatigue associated with driving long distances between the medical center and the patient's home, and pet dander. Some of the work is sedentary involving research, calculating diets, preparing reports, etc.  It may involve considerable walking throughout the medical center, prolonged standing, and carrying lightweight objects.  Work is performed in the office, medical center, the patient's home, and supermarkets.  The home setting may be hot, cold, have unsound floors, bug infestation, etc., which requires special home visit safety precautions.

BASIC REQUIREMENTS

Citizenship.  Citizen of the United States. 

Education.  The individual must have earned a minimum of a bachelor’s degree from a U.S. regionally accredited college or university, and fulfilled each of the following criteria:

  1. Completed a didactic program in dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics(ACEND), formerly the Commission on Accreditation for Dietetics Education (CADE).
  2. Completed a CADE/ACEND accredited supervised practice program.
  3. Passed a national examination administered by the Commission on Dietetic Registration (CDR), the credentialing branch of the American Dietetic Association (ADA).

Registration.  All applicants must be registered with the CDR. **

** Non-registered applicants who otherwise meet the minimum requirements in the basic qualification standard may be appointed as a graduate dietitian under the authority of 38 U.S.C. § 7405 (a) (1) (D). 

Physical Requirements.  Light lifting, under 15 pounds; Light carrying, under 15 pounds; Use of fingers; Both hands required; Near vision correctable at 13” to 16” to Jaeger 1 to 4; Far vision correctable in one eye to 20/50 and to 20/100 in the other; Hearing (aid permitted); Mental and emotional stability

English Language Proficiency.  Dietitians must be proficient in spoken and written English.

 

This position may be filled at the GS-7, GS-9 or GS-11 grade level.  A Professional Standards Board of your peers will review your application, education, and experience to determine your grade level for appointment consideration. 

 

GRADE DETERMINATIONS

In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates:

 

GS-7

Experience or Education – None beyond the basic requirements. 

KSAs – In addition, the candidate must demonstrate the following Knowledge, Skills, and Abilities:

  1. Ability to provide nutrition counseling to patients, family members and/or caregivers considering psycho-social issues and age appropriate guidelines, and to communicate effectively with other health care professionals.
  2. Ability to plan and organize work consisting of multi-tasks and priorities under general supervision.
  3. Basic knowledge of computer operations; software, hardware, terminology, security requirements, and capabilities.
  4. Ability to synthesize nutritional assessment components to evaluate nutritional status and utilize principles of medical nutrition therapy using established practice standards and guidelines to deliver nutritional care.
  5. Knowledge of basic food production and service which incorporates principles of sanitation, safety, and menu planning within a healthcare environment.
  6. Knowledge of educational concepts, methods, training materials, and resources and the ability to implement existing education and training activities for Nutrition & Food Service employees. 

 

GS-9

Experience or Education – At least 1 year of experience at the next lower level that demonstrates the core competencies described at that level, or education equivalent to 2 full years of progressively higher level graduate education, or master’s or equivalent graduate degree from an accredited university or college in the field of dietetics or closely related field. 

KSAs – In addition, the candidate must demonstrate the following Knowledge, Skills, and Abilities:

  1. Ability to plan and organize work of considerable difficulty and responsibility under minimal supervision.

For Clinical Dietitian Positions, the following Knowledge, Skills, and Abilities must be demonstrated:  

  1. Ability to participate in research and performance improvement studies.
  2. Knowledge of, and ability to, apply the basic principles of enteral and parenteral nutrition support.
  3. Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy in order to plan, develop, and independently coordinate medical nutrition therapy intervention, interdisciplinary care plans, and nutrition education activities. 
     

GS-11

Experience or Education – At least 1 year of experience at the next lower level that demonstrates the core competencies described at that level, or education equivalent to 3 full years of progressively higher level graduate education, or a Ph.D. or equivalent doctoral degree from an accredited university or college in the field of dietetics or closely related field. 

KSAs – In addition, the candidate must demonstrate the following Knowledge, Skills, and Abilities:  

  1. Knowledge of basic investigative processes and techniques in order to participate in the design of studies, collect data, interpret findings, and translate results into written and oral communication.

For Clinical Dietitian Positions, the following Knowledge, Skills, and Abilities must be demonstrated: 

  1. Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy for patients with complex medical conditions in order to plan, develop, and coordinate medical nutrition therapy intervention, interdisciplinary care plans, and nutrition education activities.
  2. Knowledge of complex areas of clinical nutrition including management of critically ill and malnourished patients, and ability to apply medical nutrition therapy to patients with multiple co-morbidities.
  3. Ability to act as a mentor/consultant for health care providers including registered dietitians, dietetic interns, nurses, physicians, medical students, and allied health professionals.
