Johnson VA Medical Center serves more than 53,000 veterans in 22 counties along the South Carolina and Georgia coastline in our main Medical Center or one of six community-based outpatient clinics.   The main Medical Center provides acute medical, surgical, and psychiatric inpatient care, as well as outpatient primary and mental health care to low country veterans who require more than 659,000 outpatient visits and approximately 4,300 inpatient stays annually.  The 117 bed hospital currently operates 96 beds and a 28 bed nursing home unit.  Charleston VAMC supports the Vet Centers in North Charleston, Myrtle Beach, South Carolina and Savannah, Georgia.

The successful candidate will function as a primary practitioner with a scope of practice to initiate, change or discontinue OTC or prescribed medication for the patients involved in the following pharmacist run programs:  Anticoagulation, Diabetes, Hypertension, and Hyperlipidemia, or for medications ordered outside of these specific areas, following discussion with the appropriate provider without co-signature (except for controlled substances).  S/he will be actively involved in the clinical pharmacy responsibilities within the Patient Aligned Care Team practice model.  The incumbent may also be responsible for medication management reviews for patients enrolled in the Conway Home Based Primary Care Program; as well as being involved in the review and processing of Non-formulary drug requests, electronic verification of medication orders, P&T therapeutic conversions, medication use evaluations, medication reconciliation, quality assurance reviews, patient counseling, healthcare provider education and additional duties as assigned.

You must demonstrate the knowledge, skills, and abilities in the following areas (The responses to the Submit the response in narrative format and submit with your application as this information will be used by the Professional Standards Board:

1.  Ability to communicate orally and in writing to persuade and influence clinical and management decisions. 

2.  Expert understanding of regulatory and quality standards for the assigned program area. 

3.  Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in the assigned program area or area of clinical expertise. 

4.  Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

5.  Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

BASIC REQUIREMENTS:  

Citizenship:  Citizen of the United States.  (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)

Education:

1)  Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.  Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at:  http://www.acpe-accredit.org/.

(NOTE:  Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program.  Today the sole degree is Doctor of Pharmacy.) 

2)  Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure.  Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.  The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.  A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.

NOTE:  Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)).  Effective November 30, 1999, this is a requirement for employment.  This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.

Physical Requirements.  All candidates must be physically and mentally able to perform efficiently the essential functions of the position, with or without reasonable accommodation, without hazard to themselves or others. Candidates must meet the physical requirements outlined in Section II and Section VI of the OPM Operating Manual, Qualification Standards for General Schedule Positions, and must possess emotional and mental stability.

English Language Proficiency.  Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

In addition to the basic requirements, candidates must meet one of the following:

Experience or Education. 

  • Candidates with at least a Master's Degree preferred
  • 1 year of experience equivalent to the next lower grade level (GS-12), or
  • Completion of a Resident or Fellow training program in a specialized area of clinical pharmacy practice

      may be substituted on a year-for-year basis.  

Preferred Qualifications:

ASHP-accredited, PGY1 residency completion.  Board Certified Pharmacotherapy Specialist or Board Certified Ambulatory Care Pharmacist highly desired.
