This position will be responsible for providing pharmaceutical care to patients seen in the ambulatory setting.   Strives to consistently provide pharmaceutical care to these patients and should optimize patient care outcomes. Supervision of Pharmacy Practice Residents and Doctor of Pharmacy students is inherently part of the responsibilities.   A limited portion of the responsibilities will be to plan and conduct research projects.   Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.   Actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions and therapeutic outcomes as required.   Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed.   Apply knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.   Must have expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.   Reviews and implements Pharmacotherapy and treatment guidelines with providers, documents clinical interventions accurately, and works collaboratively with clinical providers to provided pharmaceutical care to selected patients.   Serves as an authority on drug usage, interactions, over-dosages, and adherence to medical staff, nursing staff, technicians and students.   Must have a good working knowledge of information resources, both written and electronic.   Provides timely and accurate responses to drug information inquires from customers.   Monitors for and reports drug errors, adverse drug reactions, allergies, and patient adherence issues. Reviews literature routinely to provide an evaluation of efficacy, safety, and cost-effectiveness for routine patient care and projects assigned.   Promotes and monitors compliance for drug therapy policies, works with providers to ensure compliance with national, VISN, and local initiatives, serves on medical center and /or VISN committees as requested.   Serves as an effective preceptor, directing and supervising pharmacy residents and students as assigned, assists in the development, enhancement, and operation of resident and student training programs, and participates in activities of interdisciplinary training and in-servicing of medical staff, nursing staff, residents, students and other allied health professionals at SAVAHCS.   Effectively instructs patients and family members in the appropriate use of medications.   Assists in planning and implementing of Pharmacy quality assurance activities.   Assists in the therapeutic recommendations for selected disease states according to best evidence or VHA and/or SAVAHCS published guidelines.   Assists in the operations of the other areas of Pharmacy Service Line including distribution and clinical work in other areas.   Responsible for guidance and direction of staff members as needed by circumstance, making appointments for patients as required following all VA rules and regulations, and assisting in designing and completing research studies involving clinical pharmacist functions.   Participates in collaborative research studies involving drug therapy with other services.   Supervises projects as required by pharmacy residents and students.

 

Work Schedule:  Monday thru Friday – 8:00 a.m. to 4:30 p.m.

QUALIFICATION REQUIREMENTS:  VA Qualification Standards Handbook 5005, Part II, Appendix G15 for Licensed Pharmacist.

BASIC REQUIREMENTS:
 
   •
U.S. citizenship.  

  • Proficient in spoken and written English.

   • Graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).  You must submit A COPY OF YOUR TRANSCRIPT(S) with your application (REQUIRED).

   • Applicant must have a full, current and unrestricted license to practice pharmacy in a state, territory, commonwealth of the United States (i.e. Puerto Rico, District of Columbia).  You must submit A COPY OF YOUR UNRISTRICTED LICENSE with your application (REQUIRED).

MINIMUM QUALIFICATION REQUIREMENTS: GS-12: Must meet Basic Requirements AND Completion of the equivalent of 1 year of creditable experience at the next lower grade level GS-11, which is directly related to the position to be filled.

OR

Completion of an ACPE-accredited Pharm.D. Program.

Examples of specialized experience would typically include, but are not limited to:

 Experience gained in practicing in an ambulatory care specialty (e.g., medicine, surgery, psychiatry, neurology, spinal cord injury, ambulatory care, rehabilitation medicine, geriatrics, medical research).

You will be rated on the following Knowledge, Skills, Abilities and Other characteristics (KSAs) based on your application for this position:

KSA -1: Knowledge of a specialized area of clinical pharmacy practice or a specialty area of pharmacy such as quality assurance/utilization review, ADP systems, drug information.

PHYSICALS REQUIREMENTS: This position may require lifting of 15-44 pounds, walking and standing of 2 - 4 hours, reaching above the shoulders, and use of fingers.

SPECIFIC PHARMACY EDUCATION:

 (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.  Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at:  http://www.acpe-accredit.org/.
(NOTE:  Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program.  Today the sole degree is Doctor of Pharmacy.)

 (2)  Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
