The incumbent is the Clinical Pharmacy Specialist (CPS) in the geriatric and extended care (GEC) pharmacy program which encompasses medication management and assisting in the development of performance improvement activities.  The incumbent will also be responsible for precepting the majority of affiliate school of pharmacy students and will be the point of contact for all affiliate school of pharmacy issues.  The incumbent will also serve as a liaison between non-VA providers, VA providers, patients, laboratory service and quality management service in regards to phamacotherapy for GEC patients.  This will encompass 60% of the individual's work time.  School of Pharmacy student issues/precepting and precepting Tuscaloosa VA Medical Center residents will encompass 37% of the individual's work time.  Other administrative duties as assigned by the Chief, Pharmacy Service will comprise the remainder of the time.  The Clinical Pharmacy Specialist is supervised by the Chief, Pharmacy Service (or designee) and guided by the Chief of Geriatrics and Extended Care Service, as well as other providers in this setting.


The CPS functions at the highest level of clinical practice, works independently under the scope of practice as defined by the medical center to directly care for patients.  A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or disscontinue medication therapy and as a consultant for intensive medication therapy management services.  This includes, but is not limited to, the following:  designing, implementing, assessing, moniroting and documenting therapeutic plans utilizing the most efficient, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine the efficacy and toxicity of medication therapy.

Basic Requirements:

(a) U.S. Citizenship; (b) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree (Verification of approved degree programs may be obtained from the ACPE website at:  NOTE:  Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy; (c) Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the U.S. or the District of Columbia; (d) Proficiency in spoken and written English and (e) must meet the physical requirements of the position.  


In addition to the basic requirements above, at the GS-13 level, the following is also required.

Completion of the requirements for GS-12 (1 year of creditable experience equivalent to the next lower grade level OR completion of an ACPE-accredited Pharm.D. program) and one (1) year of experience equivalent to the GS-12 grade level.


Pharmacists in this assignment must demonstrate the following knowledge, skills, abilities and other characteristics (KSAOs):


1 . Knowledge of professional pharmacy practice.

2.  Ability to communicate orally and in writing to both patients and healthcare staff.

3.  Knowledge of laws, regulations and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

4.  Ability to communicate orally and in writing to persuade and influence clinical and management decisions;

5.  Expert understanding or regulatory and quality standards for the program area;

6.  Ability to solve problems, coorrdinate and organize responsibilities to maximize outcomes in the assigned program area or area of clinical expertise;

7.  Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

8.  Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.


Applicants with the following qualificationscharacteristics are preferred:


1.       Previous teaching experience or teaching certification

2.       Completed a second year post graduate pharmacy residency program in geriatrics, psychiatry, etc.

3.       Certification in a pharmacy program specialty such as Certified Geriatric Pharmacist (CGP), Board Certified

        Pharmacotherapeutic Specialist (BCPS) or Board Certified Psychiatric Pharmacist (BCPP).

4.       Experience in geriatrics and/or psychiatry
