DUTIES: VA Palo Alto Health Care System (VAPAHCS) has an immediate opening for an Ambulatory Care Clinical Pharmacist at the Stockton Outpatient Clinic.  This pharmacist will be integral to the provision of care and services within the Patient Aligned Care Team (PACT) model.  Specific responsibilities include:

Direct Patient Care: Will provide direct patient care (co-management) for patients’ with chronic disease states that require active medication management and assessment.  This pharmacist will monitor medication effects, interview patients, perform basic physical assessments, make dosing adjustments under an approved scope of practice, order laboratory tests, confer with the medical team members as needed, document clinic visits to include the assessment and plan,  arrange follow-up appointments, and provide education to patient and/or caregiver. Care will be provided through office visits, group visits (e.g., educational, shared medical appointments) and non-face-to-face interactions utilizing telephone, secure messaging, and telemedicine.  Common chronic disease states covered under the scope of practice include: Anticoagulation, Diabetes, Hypertension, and Hyperlipidemia.  Additional disease states are tailored to the pharmacist’s practice area and expertise. This pharmacist will be embedded with the treatment team and will actively participate in team meetings, huddles, and collaborative.   

Education: Will provide consultation to physicians, nurses and other health professionals about drug therapy (indications, efficacy and dosage, mechanism(s) of action, major contraindications and potential side effects, and adverse reactions) in support of national, VISN, and local medication use policies and performance measures. Will precept pharmacy students and PGY-1 pharmacy residents on the ambulatory care rotation.

Prescription Processing:  Will dispense urgently needed prescriptions to patients via an on-site dispensing machine, during limited designated hours.

BASIC REQUIREMENTS:  

a. Citizenship.  Citizen of the United States.

b. Education.

(1)  Graduate of a degree program in pharmacy from an approved college or university.  The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP). 

(2)  Graduates of foreign pharmacy degree programs, or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. 

c. Licensure.  Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the Unites States (i.e., Puerto Rico), or the District of Columbia.  The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. 

 

EXPERIENCE: Completion of the equivalent of 1 year of creditable experience at the GS-11 grade level which is directly related to the work of the position to be filled, or completion of a post-Pharm.D. American Society of Health-System Pharmacists (ASHP) accredited Residency.

1. COMPREHENSIVE PHARMACEUTICAL KNOWLEDGE, SKILLS, AND ABILITIES.

2. ABILITY TO ENSURE SAFE AND EFFECTIVE MEDICATION MANAGEMENT IN DIRECT PATIENT CARE.

3. ABILITY TO ESNURE ACCURATE MEDICATION ORDERS.

4. ABILITY TO PROVIDE ACCURATE DRUG INFORMATION AND TRAINING.
