MAJOR DUTIES: Primary Care Pact Team is recruiting for 1 RN Part-time Position, 40 Hours per pay period.
NOTE: Assignment to other areas of the service line or medical center may be required. While the RN reports to the Primary Care Nurse Manager and works in collaboration with his/her assigned CORE team, assignments may be made to other areas for purposes of cross coverage.
The incumbent functions as a registered nurse for Patient Aligned Care Team(PACT) program. The RN uses the nursing process and evidence-based practice to collaborate with the Core Team (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician) and Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN is a member of the core team. The RN is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN demonstrates leadership by serving as an advocate for patients, team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice.
WORK SCHEDULE: Part-time Days, M-F. Nights, Weekends, and Holidays may be required.
As a Staff Nurse you will provide prescribed medical treatment and personal care services to ill and injured veterans. Duties include documenting observations, assessments, and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes; providing a full range of nursing care to patients with a variety of physical and/or behavior problems; and accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Our facility is seeking individuals who possess good organizational skills which enable completion of work, a demonstrated ability to assume leadership in a patient centered program, and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team.
Relocation expenses will not be reimbursed.

- Citizen of the United States.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
- Must be proficient in spoken and written English.
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.
