The primary purpose of this position is to provide pharmaceutical care to all veteran patients receiving care at the VA North Texas Health Care System (VANTHCS). This includes, but is not limited to, interpretation and verification of inpatient and outpatient medication orders (unit dose and intravenous orders, and outpatient prescriptions, non formulary and medications restricted to service review). The Inpatient Pharmacist is also responsible for prospective patient profile review, obtaining and documenting allergy and adverse drug reaction information in the current computerized patient record system, as well as insuring maximum control and safety in the preparation and dispensing of all medication orders received.
Duties:
The inpatient Pharmacist will organize daily workload to assure optimum, accurate and efficient completion of all medication orders, inpatient, discharge, and outpatient in assigned areas. He/she will provide quality control checks and analysis of automated dispensing equipment as assigned; fill and issue ward or clinic orders for drugs, supplies and narcotics as necessary, and will maintain records accurately. The incumbent will provide pertinent information concerning the appropriate use of drugs to physicians, nurses, and other associated health care professionals and providers of this Medical Center. He/she will also serve as a liaison between the inpatient areas, primary care clinics, or other patient care areas and the centralized pharmacy distribution area, identifying and correcting problems and improving communication. Each pharmacist will be assigned to provide pharmaceutical needs to medicine, surgery, psychiatry, neurology, spinal cord injury, opioid replacement and/or geriatric patients. The Inpatient Pharmacist must possess in depth knowledge of one or more of these specialized areas to effectively carry out the daily responsibilities. He/she will monitor and assess drug therapy outcomes including interpretation of laboratory and diagnostic data, and provide appropriate drug therapy recommendations.
WORK SCHEDULE:
Monday - Friday (2:30pm - 11pm or 3:30pm - 12midnight) rotating weekends and holidays.

BASIC QUALIFICATION REQUIREMENTS:
Candidates must meet all the requirements identified in the VA Qualification Standard for Licensed Pharmacist, GS-660 as set forth in VA Handbook 5005/55, Part II, Appendix G15 dated June 7, 2012.
1. Graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP). Verification of approved degree programs may be obtained from the American Council on Pharmaceutical Education, 311 West Superior Street, Suite 512, Chicago, Illinois 60610-3537 Phone: (312) 664-3575), or through their Web site at: http://www.acpe-accredit.org/.
2. Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. This finding may be based on any of the following:
a. A letter of acceptance into a United States graduate pharmacy program recognized by the ACPE.
b. Written certification from the Foreign Pharmacy Graduate Examination Commission, 700 Busse Highway, Park Ridge, IL 60068, Phone (847) 698-6227, that the individual has successfully passed the Foreign Pharmacy Graduate Examination.
c. A letter from a United States college or university with a pharmacy degree program recognized by ACPE stating that the individual's pharmacy degree has been evaluated and found to be equivalent to its Bachelor of Pharmacy degree or higher.
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.
Physical Requirements. Use of fingers and both hands required. Walk (up to 1 hour); light carrying (up to 10 lbs.). Near vision correctable at 13' to 16'; hearing aid permitted.
Experience. Completion of the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled or completion of an ACPE-accredited Pharm.D. program.
Clinical Pharmacist.
A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
