The Home Based Primary Care (HBPC) is an interdisciplinary team program that is provided to Veterans in their homes. These Veterans have complex health care needs for whom routine clinic-based care is not effective. The Certified Registered Nurse Practitioner (CRNP) or Physician Assistant (PA) utilizes his/her clinical skills to facilitate, coordinate, and deliver comprehensive care to a panel of 90 Home Based Primary Care Veterans. These duties include; making home visits annually and as needed, assuming primary medical responsibility for his/her assigned Veterans, identifying Veterans' medical problems and defining medical management, prescribing medications and treatments in accordance with their individual scope of practice, making clinical decisions regarding Veterans' care based on reports from HBPC RNs calling from Veterans' homes, determining the need for consultation from subspecialty clinics, and determining the need for and facilitating admission to hospitals.
The duties listed are not meant to be all-inclusive, but merely intended to be illustrative of the nature of work to be performed by a person in the position.
Work Schedule: Monday through Friday, 8:00am to 4:30pm.
Salary Range for CRNP: $69,009.00 to $115,003.00 per year
Salary Range for PA: $37,502.00 to $96,104.00 per year
