A Critical Care Medicine physician ("Intensivist") or Pulmonary & Critical Care Medicine physician ("Pulmonary Intensivist") at the Portland VA Medical Center is assigned as a clinician-educator to the Section of Pulmonary/Critical Care. As assigned by the section chief and consistent with the VISN20 and DHSM time allocation models, clinical duties of the Intensivist will include:
1. Direct, comprehensive care of inpatients admitted to the medical intensive care unit.
2. Performance of critical care procedures including, but not limited to, airway management including endotracheal intubation, central venous catheter placement, arterial catheter placement, thoracentesis, tube thoracostomy.
3. Supervision of residents, fellows and medical students from Oregon Health & Science University (OHSU) and other affiliates in the direct care of critically ill patients.
4. Supervision of Respiratory Therapists in the medical intensive care unit
5. Close collaboration with MICU nursing and pharmacy staff to sustain a high-functioning interdisciplinary care environment.
6. In rotation with other MICU physician staff, nighttime and weekend home call for medical intensive care.
7. Indirect patient care: communications with providers at VAPORHCS and at outside institutions regarding the indications and appropriateness of transfer of Veterans to the Portland VA medical intensive care unit.
Clinical duties of the Pulmonary Intensivist will include, in addition to the above:
1. Outpatient Pulmonary consultation clinics
2. Inpatient Pulmonary consultation
3. Precepting of Pulmonary fellows and internal medicine residents in both inpatient consultation and outpatient Pulmonary clinic
4. Creation and management of templates and order sets to facilitate non-face to face care (consults, e-consultation).
5. Consult review, triage, and completion of electronic consults (E-consults)
6. Telemedicine and outreach clinics
7. Performance of pulmonary procedures including, but not limited to, flexible bronchoscopy, thoracentesis, tube thoracostomy.
a. Training in advanced bronchoscopic techniques such as endobronchial
ultrasound (EBUS), Super-Dimension electromagnetic navigation bronchoscopy, and
airway stent placement is highly desirable but not required.
8. Interpretation of Pulmonary function tests including cardiopulmonary exercise testing
9. Supervision of technicians in the Pulmonary function laboratory
10. In rotation with others, nighttime and weekend home call for Pulmonary consultation
11. Indirect patient care: returning telephone calls, responding to patient emails, reviewing and renewing prescriptions, reviewing clinical data, communicating results to patients, communications with primary care providers and other specialists, as appropriate, and implementing improved quality in systems delivery.
An Intensivist with appropriate skill and experience will
have the opportunity to assume a leadership position in Medical Critical Care
at the Portland VAMC. A Pulmonologist with appropriate skill and experience
will have the opportunity to assume a leadership position in Respiratory Care
Services and Pulmonary Function testing at the Portland VAMC.
Work Schedule: Day tour plus on call hours

Physical Requirements: This position will require walking, standing or sitting for periods of up to or beyond 10 to 12 hours each day. Under urgent or emergency situations, the incumbent may need to run, sprint, or run up or down stairs.
Citizenship.
Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens)
Education.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
(1) Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
(2) Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) of the American Osteopathic Association for the year in which the degree was granted.
(3) For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant.
Licensure and Registration.
Physicians must possess a current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. The physician must maintain current registration in the State of licensure if this is a requirement for continuing active, current licensure.
English Language Proficiency.
Physicians appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
