Position Summary:
The Mather Outpatient Behavioral Health Nurse Practitioner (NP) is a member of an interdisciplinary team that provides recovery oriented, Veteran centered clinical care for Veterans with a variety of medical, surgical, and psychiatric diagnoses in the outpatient clinic setting. The Mather Outpatient Behavioral Health NP: Is responsible for the triage and temporary medication management of Veterans in transition (i.e., medication refills); provides triage, crisis intervention services and facilitates admission to psychiatric inpatient services when needed. Collaborates with Veterans, families and interdisciplinary treatment team members in the comprehensive assessment of biopsychosocial treatment needs of Veterans. Evaluates and interprets medical data to formulate a plan of care based on the H&P and diagnostic findings. Develops treatment plans with specific goals and interventions unique to each Veteran. Facilitates laboratory tests, procedures, non-invasive tests, consultations and medications under the supervision of the program physician/psychiatrist. Prescribes medications to include controlled substances based on standardized procedures or protocols and/or physician consultation. Prescribes buprenorphine with current training and waiver. Provides ongoing medication management, individual, group and family psychotherapy. The NP is responsible for required clinical records, evaluation and monitoring of service delivery and outcome. Participates in nursing and medical center committees and task forces. Participates in patient and family education. Facilitates educational process for students by serving as preceptor. Acts as consultant and resource person to outside agencies in the area of psychiatric nursing care delivery. |
