The VA Portland Health Care System (VAPORHCS) is a Complexity Level 1A Facility and provides extensive primary, secondary, tertiary, and quarternary care in outpatient, inpatient, residential, and community settings. Portland VA Health Care System (VAPORHCS) is a multi-site medical system that includes a major medical facility in Portland, Oregon, outpatient and residential facilities in Vancouver, Washington, and ten Community Based Outpatient Clinics. The Home Based Primary Care (HBPC) Social Worker is assigned to Home & Community Based Services Department in the Rehabilitation & Long Term Care Division.
The incumbent provides Veterans in the HBPC Programs with high quality case management, clinical treatment, advocacy, and coordinates linkage with appropriate VA and community service providers/agencies as needed by the client. Services provided include short- and long-term interactions through HBPC programs including local and rural HBPC teams. This is accomplished in collaboration with other members of the HBPC Team or other interdisciplinary treatment teams as appropriate (i.e. Specialty Clinics, PACT, CLC, etc.).
Work Schedule: Monday - Friday 08:00 to 4:30 pm or Monday - Friday 7:30 am to 4:00 pm.
Preferred Experience: Minimum of 3-5 years experience, and Home Health Care experience.

Physical Requirements: The work is sedentary; however, there may
be some walking, standing, bending, carrying of light items such as

GRADE DETERMINATIONS:
GS-11 Requirements: This is the full performance level for social workers. GS-11 Social Workers are licensed or certified to independently practice social work (LCSW). They are assigned to all program areas, including inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area.
SPECIALIZED EXPERIENCE FOR THE GS-11 LEVEL: Requires a minimum of one year of post-MSW degree professional social work experience in the field of health care social work. To be creditable, the experience must have required the use of knowledge, skills, abilities, and other characteristics associated with current professional social work practice. Experience and education satisfying this requirement must be active professional practice, which is paid/non-paid employment as a professional social worker, as defined by the appropriate state licensing board. Experience must have been obtained AFTER earning a Master's Degree in Social Work.
