This position is located at the Minneapolis VA Health Care System in the Extended Care & Rehabilitation Service Line, Community Living Center (CLC), unit 1E. CLC is a hospital based transitional care area serving veterans for an extended but limited time. 1E is a rehabilitation unit with nursing staff that include RN's, LPN's, and Nursing Assistants. Common diagnoses treated on unit 1E include but not limited to: COPD, Diabetes Mellitus, Cancer, Congestive Heart Failure, Peripheral Vascular Disease, Depression, Decubitus Ulcers, Amputees, Orthopedic, and Hospice & Palliative Care. Services address the full range of functional, physical, cognitive, psychosocial and leisure needs typically exhibited by individuals requiring rehabilitation. The goal of nursing care and the interdisciplinary team on 1E is to treat residents and the prevention of complications associated with disease and disorder; performing diagnostic and therapeutic procedures; educating resident/family to improve on self-care abilities, health maintenance, and rehabilitation.
The inpatient Staff Nurse in CLC works rotating tours of duty including weekends and holidays to meet unit and patient/resident care needs.
· Utilizes the nursing process in managing, developing, implementing and evaluating each resident's individual plan of care with the interdisciplinary team to attain specific clinical outcomes.
· Provide a full range of nursing care to residents with a variety of physical and/or behavior problems.
· Demonstrates leadership in delivering and improving holistic care through collaborative strategies with
· others.
· Participates in quality improvement activities, and the evaluation of caregiver issues.
· Assesses learning needs of veterans, caregivers and staff, and implements appropriate teaching plans.
· Lead group activities for veterans, caregivers and staff.
· Works collaboratively with all levels of healthcare professional support staff to facilitate positive resident care outcomes.
· Ability to establish effective communication with veterans, caregivers, visitors and members of the interdisciplinary team
· Guides and directs resident care delivery of support staff.
