PRIMARY CARE NURSE PRACTITIONER
ENDOCRINOLOGY
APRN STATE LICENSURE AND CERTIFICATION IS REQUIRED
The Nurse Practitioner (NP) in the Endocrinology Service will have the primary responsibility of comprehensive management of patients with complex diabetes and will direct the insulin pump program. The NP will also provide consultative care to selected Endocrine patients, as available beyond his/her diabetes caseload.
The provider will be proficient in managing patients with diabetes, including insulin pump therapy and real time continuous glucose monitoring (CGM). He/she will direct the insulin pump program, working closely with the Endocrinology staff physicians and Diabetes Team to provide expert management, education and evaluation. The provider will utilize evidence based practice and current medical guidelines to promote best practices for diabetes management throughout the VAHCS. The provider collaborates closely with the Endocrinologists, Primary Care providers, and colleagues across service lines, disciplines and in the community to provide quality care to patients presenting with acute and chronic conditions. The NP will manage a panel of complex diabetic patients in the outpatient setting.
SCOPE OF ASSIGNED DUTIES
The primary focus of the NP in Endocrinology will be to provide comprehensive management of the complex diabetic population, including oversight of the insulin pump program to improve healthcare outcomes across services within the medical center for this population. The NP will offer support to the facility Certified Diabetes Educators (CDEs), who provide Diabetes Case Management to Veterans. He/she will be knowledgeable about CDE and RN use of the Outpatient Diabetes Case Management Protocol. The NP will be an expert in complex diabetic management, educating and mentoring staff.
The NP will:
· Conduct comprehensive assessments of the patients' medical, physical and biopsychosocial status including:
§ Obtains a relevant health and medical history.
§ Performs a physical exam based on age and history.
§ Orders and interprets appropriate diagnostic procedures and laboratory tests.
§ Identifies and prescribes appropriate pharmacological and non-pharmacological agents.
§ Identifies the needs of the individual and family based on the evaluation of collected data.
§ Provides relevant patient/family education.
§ Determines effectiveness of interventions based on client care outcomes.
§ Manages new and unstable insulin pump patients.
§ Interprets CGM data and adjusts the treatments as indicated.
§ Manages chronic endocrine patients.
· Provide direction and oversight for the Insulin Pump Program at the Minneapolis VA Healthcare System
§ Approves all new initiation of CGM therapy based upon VHA guidelines.
§ Assesses and approves recommendations from facility CDE's for new insulin pump initiation.
§ Develop (as needed) and review policies necessary for the safe and effective use of insulin pump
therapy, CGM and its associated technology.
§ Provide expert clinical support for the CDEs in difficult to manage cases.
§ Periodically review and update Outpatient Diabetes Case Management in collaboration with the Endocrinology staff physicians, facility CDE's and Diabetes Advisory Committee.
§ Serve as facility liaison to insulin pump manufacturer and community resources.
§ Engage in quality assurance projects as needed to ensure sustained quality of care for this high-risk
population.
· Serve on the facility Diabetes Advisory Committee
· Serve as a resource to members of the health care team, providing consultation and education as needed.
· Actively participate in Department level quality improvement processes and initiatives.
· Perform any procedures for which she/he has been credentialed

Grade Determinations:
Education/Experience Requirements: A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a masters degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body.The certification must be in the specialty to which the individual is being appointed or selected.
