The Certified Registered Nurse Practitioner (CRNP) provides diagnosis, treatment, consultation, and follow-up under the supervision of the Nurse Manager in the Medicine Service Line at the Pittsburgh VA Medical Center.
The incumbent CRNP will be stationed in our 18 slot chemotherapy infusions clinic. The incumbent will communicate directly with the doctors, pharmacists, and nurses regarding implementation of treatment from pre-initiation planning thru completion of all scheduled chemotherapy cycles. The CRNP will perform triage in the infusion clinic setting and be the primary point of contact concerning treatment related issues for patients actively receiving chemotherapy. The CRNPs reviews test results, assesses patient for side effects, monitors pain control and manages all supportive care medications based on patient needs at the time of their chemotherapy infusion appointment. This is not a managerial position; the CRNP reports directly to the Nurse Manager.
The incumbent will:
1. Administer direct physical care to selected patients/families.
2. Independently assess acute and non-acute clinical problems and treatment toxicity.
3. Reviews treatment plans and prescribes interventions or treatments which may include the prescriptions of medication or ordering of procedures/images as needed.
4. Case manages complex oncology patients to assess, monitor and evaluate care, i.e., chemotoxicities, comorbidities, and psychosocial issues.
5. Provide ongoing evaluations of care management patients; works closely with attending physicians, fellows, Primary Care and other specialties.
6. Assess and coordinate treatment for symptom management, and/or refer to appropriate specialty for higher level care needed.
7. Assess and link patient to additional services when needed, i.e. alcohol treatment, nutrition, palliative care, hospice as necessary.
8. Assists the Nurse Manager in coordinating patient care operation at the unit level.
9. Monitor and coordinate efficient patient flow through the infusion clinic.
10. Collaborates with charge nurse to make patient/staff assignments considering competency of the staff and patient flow. Helps to ensure assignments are fair, equitable and in accordance with patient acuity, needs, etc.
11. Role models collaborative leadership and communicates effectively.
12. Provides clinical guidance and support to RNs and MSAs in the Chemotherapy Clinic.
13. Participation in unit/program level quality improvement processes, initiatives and customer service programs.
