Located in the Outpatient Care Service Primary Care at the Mann-Grandstaff VA. The Licensed Practical Nurse (LPN) is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN demonstrates active collaboration and communication with the Core Team (Patient, Primary Care Provider, RN Care Manager and the Clerical Associate) and the Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). The LPN must have the ability to react to situations in accordance with established policies and procedures, and ability to appropriately use this knowledge to work effectively with a wide range of patients.
Duties include:
1. Makes observations and thoroughly documents the patient's care and condition. Appropriately seeks assistance and guidance by informing the Provider or RN of any condition requiring higher-level intervention.
2. The LPN performs, reports and records assigned nursing care in the Primary
Care Clinic, including, but not limited to:
a. Oxygen therapy
b. Medication Administration
c. Clinical Reminders with appropriate consults and alerts, as indicated.
d. Check expiration dates of medications and supplies
e. Delegated procedures and treatments
f. Documentation in electronic medical record
g. Assist with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy.
3. Adheres to standards of practice and safe techniques in preparation and administration of medication.
4. Demonstrates understanding of age-related and population-specific characteristics that affect the health care needs of the Veteran.
5. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed.
6. Provides patient and family health education, to include self-management, prevention and wellness, utilizing the patient's stated goals.
7. Interacts and communicates appropriately with patients and family members.
8. Demonstrates professional behavior (respect, civility, ability to accept constructive criticism), effective communication, teamwork and interpersonal skills.
9. Adheres to the principles of infection control, safety, and universal precautions.
10. Possess knowledge of proper handling and pre-cleaning of reusable medical equipment.
11. Demonstrates initiative in assisting the teamlet and the larger teams.
12. Follows policy and procedures related to fire and disaster drills, safety and security measures.
13. Demonstrates competence utilizing special equipment needed to perform job duties.
14. Assists in the orientation and competency validation of students, peers and clerical staff.

Physical Requirements:Work requires the LPN to spend most of the tour on his/her feet with intermittent walking, bending, pushing, pulling and lifting up to 10 pounds. The employee is required to safely use mechanical devices and proper body mechanics for lifting. The employee accepts assignment on an approved tour of duty in the assigned area where service is required. Work requires the ability to use a keyboard to access, input and retrieve information using a computer with keyboard. Work requires finger and hand dexterity adequate to handle and manipulate equipment, stocking of supplies, submitting reports and filing forms.
Basic Requirements:
Citizenship. Citizen of the United States.
Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing [Accrediting Commission(NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the National League for Nursing [Accrediting Commission], 61 Broadway, 33rd Floor, New York, NY 10006. Additional information may also be obtained from the NLNACWebsite. Exceptions include:
(1) Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
(2) VHA employees who were converted to appointments under 38 U.S.C.7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3)or 7405 after these conversions.
(3) Individuals granted a license by a jurisdiction that does not require graduation from an approved school. See paragraph 4, section C.
Licensure. Full, active, current and unrestricted licensure as a LPN/LVN in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Grade Determinations: The full performance level for a LPN/LVN is GS-6. - applicants must qualify at this level GS-6: Total required experience for the GS-6 is 2-2.5 years of LPN experience
Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.
