This position is based out of Medical Services, Northern Arizona VA Medical Center, Prescott, AZ but is to include telework. The position utilizes a Clinical Video Telehealth (CVT) modality to deliver primary care services to patients in other clinics in VISN 22 and VISN 17.
You will report directly to the Tele-primary care Hub Director or Section Chief within the Medical Service. Program goals and objectives are provided. As an independent practitioner, the incumbent is responsible for planning and carrying out the assignment independently, resolving most of the conflicts that arise, coordinating the work with others as necessary, and interpreting policy on own initiative in terms of established objectives. GENERAL DESCRIPTION OF ASSIGNED DUTIES:
The major duties of the position include, but are not limited to:
There are back-up services available in the local an VA emergency rooms (near the remote clinic site) as well as surgery, psychiatry, and medical subspecialties )affiliated with the remote clinic site.) Patients who need admission to the inpatient wards will be transported to the nearest VA facility when appropriate. The physician may sometimes need to work alone, but can request assistance from other medical staff (including other telehealth primary care staff, as well as staff in the Emergency Room or inpatient service at the nearest VA medical center when needed.) Shifts may occasionally deviate from the usual 8:00am to 4:30pm shift schedule. The facility is fully computerized and computer skills are required. Special training will be provider in order to utilize CVT technology.
The is minimal administrative work, but the physician will be required to attend staff meetings, team meetings, some committee meetings and complete some peer review evaluations. Time will be provide outside of assigned shifts to complete this work.
SCOPE OF ASSIGNED DUTIES:
The physician will provide direct patient care full time and work as a member of an expanded healthcare team (social worker, dietician, clinical pharmacist, behavioral health clinician, and a nurse care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting utilizing the Patient Aligned Care Team model. Health Administration Service and Nursing staff will usually be provided by the remote clinic location/facility.
The practice emphasizes evidence-based medicine, primary and secondary preventative medicine, the use of computerized medical record system and continuous quality improvement based on individualized provider data.
Work Schedule: 1st Shift 8:00 am to 4:30 pm
