This co-located collaborative care (CCC) Social Worker position is administratively within the Mental Health Service Line. The CCC mental health social worker health functions as part of multidisciplinary primary care teams, and delivers brief consultation-based services to Veterans, primary care providers, and allied Patient Alliance Care Team (PACT) members. The focus is on general service delivery for a wide range of concerns and resolving problems within the primary care service context. PCMHI visits are brief (generally 20-30 minutes), limited in number (1-6 visits), and are provided in the primary care practice area. CCC providers function within fast-paced primary care teams and can expect to consult with 8-10 patients per day with the primary goals of assisting PACT members with identification , treatment, and management of mental health and behavioral medicine conditions in the enrolled population.
The PCMHI Program Coordinator is an integral member of the Primary Care team and works in conjunction with primary care and mental health leadership to coordinate administrative, patient, and personnel activities for this program. (S)He is administratively responsible for clinical programming and the daily operation of the program, as well as, managing the data related to the program
The Social Worker in this position requires an advanced level of knowledge and understanding of the full spectrum of mental health problems, chronic medical illness and health interfering behaviors. The incumbent must have highly developed skills in collaboration, consultation, and evaluation of efficiency and efficacy of program components.

GRADE DETERMINATIONS:
Social Worker Program Coordinators and Manager. For Social Worker Program Coordinators, grades are determined by the scope of responsibility and the type, size, and complexity of the assignment.
Experience, Education and Licensure. Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-masters degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below.
Demonstrated Knowledge, Skills, and Abilities
1. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
2. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.
3. Ability to supervise multidisciplinary staff assigned to the program.
4. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).
5. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.
Assignments: The KSAs shown above apply to all Social Worker Program Coordinator grade levels.
GS-12 Social Worker Program Coordinators. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, such as Spinal Cord Injury (SCI) coordinators, Visual Impairment Services Team (VIST) coordinators, Homeless Veterans Program (HVP) coordinators, Community Nursing Home Program (CNHP) coordinators. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the programs budget.
