Medical Foster Home (MFH) is a type of Community Residential Care (CRC) Home Based Primary Care (HBPC), operating under the Community Residential home program which is chosen by the Veteran who is unable to live independently. The Medial Foster Home Coordinator is a healthcare professional who is responsible for the management and oversight of day to day operations of the Medical Foster Home Program (MFHP). The position is located in the Primary Care Service of the Jack C. Montgomery VA Medical Center, Muskogee, OK. The incumbent will be serving both the VA Medical Center in Muskogee, OK and the CBOC, in Tulsa, OK. Some of the goals to accomplish these task include, but are not limited to:
Work Schedule: Monday through Friday, 8:00 am to 4:30 pm
Functional Statement Title/#: Social Worker Program Coordinator-Medical Foster Home Program (MFHB)
Physical Requirements: Walking (2-4 hours); Standing (2-4 hours); Repeated bending (1-2 hours); Operation of crane, truck, tractor or motor vehicle; Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16"; Far vision correctable in one eye to 20/20 and up to 20/40 in the other; Hearing (aid permitted); Emotional Stability.

In addition to meeting the basic requirements, applicants must have six months of specialized experience. Specialized experience is a minimum of 6 months experience working in a clinical care area to coordinate patient care activities. At a very minimum, applicants must possess the following preferred characteristics:
Grade Determinations:
GS-12 Senior Social Worker - In addition to the Basic Requirements, GS-12, Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgement. At least two years should be in an area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (exept for licenses issued in California, which administers its own clinical examination for advanced practice). Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Social Worker Program Coordinators and Manager - Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progrssively more professional competency. They may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
