The Non-Institutional Care (NIC) Coordinator - Registered Nurse (RN) is a professional nurse who provides leadership and assumes continuous responsibility for the development, implementation, coordination, maintenance and evaluation (intensive case management) of highly vulnerable patients who are at risk for institutional care. The NIC Coordinator assesses the appropriate services for the veteran based on needs, the recommendations of the VA provider, and the availability of contracted services; reviews the plan of services; is in regular contact with the agency providing care and the veteran in the program for the evaluation of need for services and to address issues (if any).
The Geriatric and Extended Care (GEC) product line offers a continuum of care that provides inpatient and outpatient short term, intermediate and long term care services and are based on the patient's need. Services include contract community nursing home, The following service categories and treating specialties are available:
· Short Stay rehabilitation, skilled nursing, restorative care, maintenance care for those awaiting alternative placement, respite
· Long Stay skilled nursing, maintenance care, spinal cord injury and disorders, and palliative care. The outpatient continuum covers rehabilitative services, home care, community adult day care and community hospice. The GEC product line covers both direct patient care and medical administrative responsibilities directed at meeting the comprehensive needs of the VA's patient population.
