Primary Care Outpatient Clinic is located in the Clinical Addition Building at the Crescenz VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA and is open during regular business hours and some extended ( evening ) hours, Monday through Saturday.
The Primary Care Clinic (PACT) "Patient Alignment Care Team" offers an extensive range of treatment modalities and a variety of health services for veterans focusing on wellness education, preventive health treatment, disease management, and the emotional well being of patients.
The Primary Care Registered Nurse is responsible for all aspects of care for patients assigned to the clinic. She/he provides general nursing care and leadership, working collaboratively with physicians and multidisciplinary team members
The Primary Care Registered Nurse is self-directed and possesses effective skills in clinical assessment, critical thinking, problem resolution, interpersonal relationships and communication. She/he demonstrates leadership in coordinating comprehensive and continuous patient-centered holistic care through active collaboration, communication, and coordination of VA and community based resources and services.
The staff nurse will promote and restore patient's health as they provide, delegate, evaluate and coordinate comprehensive professional nursing care through the use of the nursing process for patients of all backgrounds; She/he provides physical and psycho-social support to patients, and families.
Tour of Duty: Eight (8) hour shifts Monday through Friday and some Saturdays. Some evening and Saturday hours based on Veteran needs..

Grade Determinations: Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows:

Physical Requirements:
Must possess mental and emotional stability. Functional requirements include the following abilities: Heavy lifting of 45 pounds and over, light carrying under 15 pound, use of fingers/ both hands are required, walking for four (4) hours, standing for four (4) hours, both legs are required, ability for rapid mental and muscular coordination simultaneously, near vision correctable in one eye to 20/20 and to 20/40 in the other eye, ability to distinguish shades of color, hear the whispered voice (aid permitted). Environmental Factors required: Working with hands in water, working closely with others and protracted or irregular hours of work.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
