This position is located at the Community Based Outpatient Center (CBOC) in Alpena, MI associated with The Aleda E. Lutz Veteran Affairs (VA) Medical Center. The pharmacist may travel to Veterans' homes during working hours and may occasionally be required to travel to other CBOCs or to Saginaw. The Clinical Pharmacy Specialist (CPS) has prescribing privileges as outlined in the individual's Scope of Practice. The primary responsibility of the CPS is the provision of evidence-based pharmaceutical care services through direct care with patients and consultation with other providers on their team. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The CPS functions as part of the Home Based Primary Care (HBPC) Patient Aligned Care Team (PACT), and is most effective when assigned a panel of patients requiring medication therapy management (MTM) services, to support continuity of care and ensure the best use of pharmaceuticals. The CPS will follow patients in direct care setting to achieve therapeutic goals identified by the primary care provider. Clinical Pharmacy Specialists are employees of the medical center's Pharmacy Service. This is to ensure they receive all professional guidance, policy and communications, along with department missions and goals, especially regarding the VA National Formulary, Veterans Integrated Service Network (VISN) 10 Medication Management Council, and Saginaw Pharmacy and Therapeutic Committee objectives for formulary management activities and patient safety updates (i.e. notices, bulletins). The CPS will be given appropriate resources and physical space (i.e. clinic room) for the direct care of their patients. The CPS will follow patients until they achieve the pharmaceutical therapeutic goals set by the provider, chief of Staff, or designee. The duties include but are not limited to:
· Is directly involved in the design, implementation and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
· Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed.
· Is able to apply knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.
· Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures.
Work Schedule: 8:00 AM to 4:30 PM (Monday through Friday)

BASIC REQUIREMENTS:
a. Citizenship. Citizen of the United States.
b. Education
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/.
(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
c. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
d. English Language Proficiency. Pharmacists must be proficient in spoken and written English.

GS-13 Clinical Pharmacist Specialist:
Experience: At least 1 year of specialized experience equivalent to the next lower grade level GS-12 directly related to the position being filled that demonstrates the clinical competencies described at that level. Examples of experience for this position include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.

Preferred Experience: Completion of a Post Graduate Year 1 and/or Post Graduate Year 2 residency program is preferred. Experience in independent disease state management including prescribing for disease states such as diabetes, hypertension, and hyperlipidemia is also preferred.
