Functional Statement Title/#: GS-0660 Clinical Pharmacy Specialist (Internal Medicine)
DUTIES:
The position functions in areas which primarily service the internal medicine teaching team of the inpatient section of Pharmacy Service. The Clinical Pharmacy Specialist is responsible for planning, developing and maintaining effective clinical pharmacy programs by implementing rational cost effective drug therapy to improve patient outcomes. In this capacity, the incumbent assists with clinical education activities, provides drug information, and contributes to MUE activities. Duties include, but are not limited to the following: Participates in inpatient rounds with internal medicine teaching team. Serves as an authoritative information source on drugs and their utilization in therapy. Evaluates the drug literature by analysis of experimental design and evaluation, in order to compare and contrast therapeutic regimens and roles for new drugs as it applies to patient care. Interfaces with physicians and treatment teams in the design of dosage regimens from current pharmacokinetics principles and specific patient parameters, and effectively communicates recommendations to the patient's provider and documents activity in the patient's medical record. Utilizes a scope of practice to design and implement a therapeutic regimen or plan based on patient specific information that includes a therapeutic endpoint, selecting an appropriate drug and dosage regimen, and providing alternative regimens in cases where the initially developed regimen fails to achieve the desire therapeutic result. Provides consultation on selection of appropriate dosage regimens as follow-up when non-therapeutic drug levels are obtained for inpatient medical/surgical patients. Initiates requests for appropriate laboratory and other diagnostic studies when needed for monitoring drug therapy, with notification of the primary provider. Performs medication reconciliation and summarizes significant findings which are documented in the medical record and reported to the primary provider. Identifies any drug-induced problem that may be affecting the patient or contributing to the need for hospitalization, including adverse effects, allergic reactions, toxicities, improper dosage regimen design, noncompliance, etc. Participates in formulary management through participation in the non-formulary drug consult service. Provides verbal and written medication information to patients or families on request and appropriate discharge counseling when necessary. Develops and provides training programs for pharmacy students and residents to facilitate continued growth of knowledge and skills which help to insure an high level of quality in professional activities.
Physical Requirements: Applicants must met the physical standards for the position. The work requires sitting, walking, standing, bending and stooping, pulling an pushing and lifting/carrying of items up to 20 to 50 lbs. and be able to maintain self-control in difficult stressful situations. A physical examination prior to placement is required.
BASIC REQUIREMENTS:
· U.S. Citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
· Education. Must be a graduate of a degree program in Pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE).
· Licensure. Must have a full, current and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States or District of Columbia.
· Physical Requirements. Must be able to pass a pre-employment physical in accordance with VA Policy.
· English
Proficiency. Must be proficient in spoken and
written English language.

Grade Determinations: Grade and salary will be determined by a Professional Standards Board (PSB). In addition to the basic requirements for employment, the following criteria must be met:
MINIMUM QUALIFICATION REQUIREMENTS:
At the GS-13 level, in addition to the basic requirements applicants must meet the following:
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Experience is only creditable if it is post-licensure experience as a professional pharmacist directly related to the position to be filled. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time work week.
Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Preferred Experience: An American Society of Health-System Pharmacists (ASHP) accredited residency or equivalent clinical experience in areas of Internal Medicine/Acute Care; Experience managing complex Anticoagulation care.
