NURSE PRACTITIONER
CARDIOLOGY WITH ELECTROPHYSIOLOGY FOCUS

APRN STATE LICENSURE AND CERTIFICATION IS REQUIRED
Advanced practice nurses (APN) in this role collaborate with Cardiologists in responding to in-patient and outpatient Cardiology consults with a focus on Electrophysiology. This position is one of a group of Cardiology Advanced Practice Nurses who respond to cardiology consult requests from within the hospital as well as from the VISN facilities. This role serves as liaison between the physicians, patients, families, associated VISNs and CBOC's. APN's in Cardiology report directly to the Nurse Manager and Chief of Cardiology. The APRN in this role provides specialty and primary care to veterans in the outpatient setting, by telephone and telemedicine. This role may also interact with the patients on the acute care, emergency room and/or the extended care wards. The population served includes both male and female veterans across the adult lifespan, as well as active duty personnel. Patients managed by the Cardiology APRN may have complex co-morbidities such as hypertension, COPD, diabetes, atrial fibrillation, valvular diseases, coronary artery disease, cerebral vascular disease, auto immune disorders and renal impairment.
Primary functions of this role includes:
· Obtain health history, perform physical exams and documents findings in the medical record.
· Orders appropriate diagnostic procedures, and acts on results
· Prescribes medication, treatments and patient supplies per practice agreement
· Educate patient and significant others on health care issues (ie. Medications, diet, life style changes and follow up).
· Facilitate the completion of informed consents prior to the procedure
· Schedules patients for labs, EKG's, x-rays, procedures and appointments
· Admits and discharges patients to OBS status post invasive cardiac procedures.
· Mentor to APRN students
· Collaborates to arrange planned procedures such as coronary catheterization, pacemaker placement, electrophysiology studies and/or ablations.
· Provide contact information to pre-procedure patients so they are accessible to answer any questions or respond to concerns pre/post procedure.
· Cardiology pre-procedure evaluation including orders, H&P and release from short stay post procedure including education and follow up appointments.
· Acts as consultant and resource to other departments within and outside the medical center regarding Cardiology.
· Prescribes individualized pharmacological and non-pharmacological interventions for treatment of heart failure and medical problems for the promotion of quality of life, wellness, and improved outcomes as demonstrated by reduction of hospitalizations and/or urgent care visits.
· Provides cardiology focused in-services and education programs to nursing staff.
· Participates in performance improvement initiatives.
· Independently sees patients in clinic.
· Provides cross coverage for Cardiology APRN's as needed.
· Participates in certain aspects of inpatient discharge planning, i.e. follow up for outpatient procedure.
· Ability to manage complex treatment and medication regimen in outpatient setting
· Ability to function independently in a busy Cardiology department setting.
· Develop, update/maintain and provide cardiology education to patients, families and significant others for the purpose of maximizing healthcare outcomes and promoting wellness.

Education/Experience Requirements: A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a masters degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body.The certification must be in the specialty to which the individual is being appointed or selected.
